Zusammenfassung
Die „Cornea Society“ gründete im Jahr 2005 ein internationales Komitee von 17 Mitgliedern aus den USA, Asien und Europa mit der Auflage, eine neue Klassifikation der Hornhautdystrophien (HD) zu erstellen, die den aktuellen klinischen, histologischen und genetischen Wissenstand widerspiegeln sollte. Die beiden Autoren sind Mitglieder des Komitees. Bei der Erarbeitung der Klassifikation mussten zahlreiche Fehleinschätzungen korrigiert werden, die in der vergangenen internationalen Literatur bei den einzelnen Formen von HD gemacht wurden. Trotz der wichtigen molekulargenetischen Ergebnisse im Hinblick auf Chromosomenlokalisation und Identifizierung des Gens und entsprechender Mutationen erfolgte die Unterteilung der HD nach der anatomisch hauptsächlich betroffenen Schicht innerhalb der Hornhaut. Für jede der über 25 verschiedenen Formen von HD wurde eine präzise und aktuelle Vorlage erstellt, die eine Zusammenfassung der klinischen, histologischen und genetischen Befunde ermöglichte. Der jeweilige wissenschaftliche Kenntnisstand einer Form von HD wurde in Bezug auf Klinik, Histologie und Genetik in 4 verschiedene Kategorien unterteilt, um den Evidenzgrad im Hinblick auf die Autonomie der betreffenden HD aufzuzeigen. In den Jahren 2008 und 2011 wurde die neue internationale Klassifikation der HD in englischer, spanischer und deutscher Sprache als 40-seitige Supplementpublikation veröffentlicht. Neben 64 meist farbigen Abbildungen findet sich im Anhang der Publikation eine detaillierte Zusammenfassung der genetischen Mutationen.
Abstract
The Cornea Society founded an international committee of 17 corneal experts from the USA, Asia and Europe in 2005. The goal of this group was to develop a new international classification of corneal dystrophies (CD) based on modern clinical, histological and genetic knowledge. Both authors are members of this committee. The elaboration of the classification included the correction of many misinterpretations with regard to the different forms of CD which were published in the past literature. In spite of important results concerning the genetic locus and identification of genes and mutations, corneal dystrophies are typically classified with respect to the level of the cornea that is involved. An accurate and up-to-date template for each form of the 25 CDs was created that included the current clinical, histological and genetic information. To indicate the level of evidence supporting the existence of a given dystrophy, the CDs are divided into four different categories. The new international classification of CDs was published in English, Spanish and German as a 40 page supplement with 64 figures, mostly in color. A more detailed description of genetic mutations is included in the appendix.
