Zusammenfassung
Hintergrund
Monochromatische Aberrationen beeinträchtigen die Abbildungsqualität des Auges. Sie sind dabei nicht konstant, sondern unterliegen zahlreichen Veränderungen und Einflüssen. In vorliegender Arbeit werden die Veränderungen der Wellenfront bei Akkommodation dynamisch untersucht.
Methode
Mithilfe des WASCA Wavefront Analyzer (Fa. Carl-Zeiss Meditec) und eines speziell entwickelten Target-Aufsatzes wurden monochromatische Aberrationen niederer und höherer Ordnung unter Nah- und Fernsicht bei 25 Probanden im Alter von 15 bis 27 Jahren gemessen. Besonderes Interesse galt dabei der dynamischen Wellenfrontanalyse.
Ergebnisse
Bei Akkommodation zeigten sich folgende signifikante Veränderungen: Neben der charakteristischen Sphärenreaktion kommt es zur begleitenden Miosis und Änderung der Wellenfrontneigung (Z1 1) im Sinne einer Konvergenzbewegung des Bulbus. Darüber hinaus steigt der Defokus (Z2 0), und der Astigmatismus (Z2 −2) verändert sich. Bei den Aberrationen höherer Ordnung findet sich eine Verringerung der Komakoeffizienten (Z3 −1, Z3 1). Die deutlichste Veränderung liegt bei der sphärischen Aberration (Z4 0) vor. Diese wird stärker und nimmt dabei negative Werte an. Darüber hinaus wachsen auch der sekundäre (Z4 −2) und der vierachsige Astigmatismus („quadrafoil“, Z4 4). Die Gesamtaberrationen („root mean square“, RMS) und auch die Abbildungsfehler höherer Ordnung („RMS higher order“, RMS-HO) nehmen bei Nahakkommodation signifikant zu; dies geht rechnerisch mit einer Verschlechterung der Abbildungsqualität einher. Bei der Analyse des Pupilleneinflusses auf die Aberrationen zeigen sich bei zunehmendem Pupillendurchmesser signifikante Anstiege von Defokus, sphärischer Aberration, Quadrafoil sowie auch von RMS und RMS-HO. Durch die akkommodationsassozierte Miosis können die wachsenden Abbildungsfehler bei Nahsicht teilweise kompensiert werden. Die zeitliche Analyse des Akkommodationsprozesses mit dynamischer Wellenfrontanalyse offenbart signifikante Verzögerungen von Pupillomotorik und Änderung der Wellenfrontneignung in Bezug zum Zeitpunkt der Sphärenreaktion. Bei Nahakkommodation findet sich für die Aberrationen 3. Ordnung ein diskreter zeitlicher Vorsprung vor der Sphärenveränderung.
Schlussfolgerung
Mithilfe der dynamischen Wellenfrontmessung gelang eine sequenzielle Auswertung der Aberrationen während des Akkommodationsvorgangs. Dabei zeigten sich signifikante Veränderungen der Abbildungsfehler niederer und höherer Ordnung. Diese werden zusätzlich durch die begleitende Pupillenreaktion variiert. Darüber hinaus konnte die Synchronität der Wellenfrontreaktion bei Akkommodation analysiert werden.
Abstract
Background
Monochromatic aberrations may influence the visual acuity of the eye. They are not stable and can be affected by different factors. The subject of the following paper is the dynamic investigation of the changes in wavefront aberration with accommodation.
Method
Dynamic measurement of higher and lower order aberrations was performed with a WASCA Wavefront Analyzer (Carl-Zeiss-Meditec) and a specially constructed target device for aligning objects in far and near distances on 25 subjects aged from 15 to 27 years old.
Results
Wavefront aberrations showed some significant changes in accommodation. In addition to the characteristic sphere reaction accompanying miosis and changes in horizontal prism (Z1 1) in the sense of a convergence movement of the eyeball also occurred. Furthermore defocus rose (Z2 0) and astigmatism (Z2 −2) changed. In higher-order aberrations a decrease in coma-like Zernike polynomials (Z3 −1, Z3 1) was found. The most obvious change appeared in spherical aberration (Z4 0) which increased and changed from positive to negative. In addition the secondary astigmatism (Z4 −2) and quadrafoil (Z4 4) rise also increased. The total root mean square (RMS), as well as the higher-order aberrations (RMS-HO) significantly increased in accommodation which is associated with a theoretical reduction of visual acuity. An analysis of the influence of pupil size on aberrations showed significant increases in defocus, spherical aberration, quadrafoil, RMS and RMS HO by increasing pupil diameter. By accommodation-associated miosis, the growing aberrations are partially compensated by focusing on near objects. Temporal analysis of the accommodation process with dynamic wavefront analysis revealed significant delays in pupil response and changing of prism in relation to the sphere reaction. In accommodation to near objects a discrete time ahead of third order aberrations in relation to the sphere response was found.
Conclusion
Using dynamic wavefront measurement achieved a sequential analysis of aberrations during accommodation. Significant changes in the lower and higher-order aberrations could be detected. These are additionally varied by the associated pupillary response. Moreover, the synchronicity of wave front reaction in the accommodation process was proven.
