Zusammenfassung
Hintergrund
Epithelimplantationszysten stellen eine seltene, aber schwerwiegende Komplikation nach penetrierendem/perforierendem Bulbustrauma oder intraokularer Chirurgie dar. Das Auftreten solcher Implantationen ist auch noch nach Jahrzehnten möglich.
Kasuistik
Im Fall einer 53-jährigen Patientin entwickelten sich 48 Jahre nach penetrierendem Trauma mit einem Dartpfeil 2 Epithelimplantionszysten am linken Auge. Nach kompliziertem, mehrzeitigem Wundverschluss nach penetrierendem Bulbustrauma war der Befund 48 Jahre stabil, ehe es zu Symptomen gekommen war. Die präoperative Diagnostik (Ultraschallbiomikroskopie) konnte den Ausgang der Epithelimplantationszysten von einer intrakorneal gelegenen Zyste nachweisen. Die Histologie bestätigte den klinischen Verdacht einer Epithelimplantationszyste.
Fazit
Bei unserem Fall handelt es sich um die längste dokumentierte Latenz zwischen Trauma und Auftreten der Epithelimplantationszysten, welche im Detail dargestellt wurde. Durch Ultraschallbiomikroskopie ist der Ausgangspunkt für Epithelimplantationszysten nachweisbar.
Abstract
Background
Epithelial invasion is a rare but severe complication after penetrating eye trauma or intraocular surgery. Cystic ingrowth can occur even after decades.
Casuistics
A 53-year-old woman developed two epithelial cysts in the left eye 48 years after penetrating trauma with a dart. After primary wound closure the intraocular status remained stable for 48 years before symptoms appeared. Preoperative diagnostics (e.g. ultrasound biomicroscopy) detected the origin of the epithelial downgrowth from an intracorneal cyst. Histology confirmed the clinical suspicion of a cystic epithelial ingrowth.
Conclusion
The latency of our case is the longest reported interval between penetrating eye trauma and appearance of epithelial ingrowth to be described in detail. Ultrasound biomicroscopy is able to detect the origin of epithelial ingrowth.
