Zusammenfassung
Ziel
Zur Therapie retinaler Venenverschlüsse wurde die radiäre Optikusneurotomie (RON) empfohlen. Wir berichten über einen Patienten, der sich mit einer hochblasigen, peripapillären, bis knapp an den Äquator reichenden Ablatio retinae 2,5 Jahre nach radiärer Optikusneurotomie bei Zentralvenenverschluss vorstellte.
Ergebnisse
Ein 69-jähriger Mann stellte sich 2,5 Jahre nach radiärer Optikusneurotomie mit einem weiteren Gesichtsfeldverfall vor. Ursache war eine ungewöhnliche hochblasige Netzhautablösung, die von der Papille nach nasal oben reichte und den Äquator nicht überschritt. Diese schien sich sehr langsam entwickelt zu haben. Die Ablösung konnte durch eine Cerclage kombiniert mit Pars-plana-Vitrektomie, Endolaserkoagulation und Lufttamponade vollständig zur Anlage gebracht werden. Da sich kein Netzhautriss fand und die subretinale Flüssigkeit auch nicht über die RON drainiert werden konnte, erfolgte die Drainage über eine kleine Retinotomie.
Schlussfolgerungen
Obwohl auch an andere Mechanismen gedacht werden muss, scheint die Netzhautablösung möglicherweise durch eine Fistel zwischen Subarachnoidalraum und subretinalem Raum entstanden zu sein. Mit einer Laserbarriere konnten wir diese Fistel schließen. Bei einer RON sollte darauf geachtet werden, dass der Schnitt nicht zu tief geführt wird, um eine solche Komplikation zu vermeiden.
Abstract
Objective
It has been suggested that radial optic neurotomy (RON) may improve the functional outcome of retinal vein occlusions. We report the case of a patient presenting with a high, but circumscribed, peripapillary retinal detachment following RON for central retinal vein occlusion.
Results
A 69-year-old man presented with a high retinal detachment 2.5 years after RON for central retinal vein occlusion. It was complicated by a recurring cystoid macular edema; however, best visual acuity was 0.5. The retinal detachment developed slowly. Its location was nasal and superior to the optic disc. It did not extend beyond the equator and had no detectable retinal break. The retinal detachment was successfully treated by an encircling band, pars plana vitrectomy, laser coagulation, and air endotamponade. There was no obvious persisting retinal defect at the RON site.
Conclusions
Although other mechanisms should be taken into account, this case indicates that it might be possible to create a fistula between the subarachnoid and subretinal space, leading to a slowly developing retinal detachment. A similar case report pointed out that special attention should be paid to the depth of the radial incision into the optic nerve head.
