Zusammenfassung

Hintergrund

Die Behandlung einer Cataracta congenita bei Säuglingen kann mittels Pars-plana-/Pars-plicata-Lentektomie erfolgen, wenn auf eine primäre Intraokularlinsenimplantation verzichtet wird. Zur Verhinderung von Nachstar wird die Linsenaspiration mit einer Kapsulotomie und anterioren Vitrektomie kombiniert. Für diesen Eingriff sollte ein möglichst atraumatisches Operationsverfahren gewählt werden, um Komplikationen zu minimieren und eine sofortige Kontaktlinsenversorgung zu gewährleisten.

Methode

Wir führten im Zeitraum von April 2007 bis September 2008 bei insgesamt 11 Augen von 8 Kindern im Alter von 6 Wochen bis 13 Monaten (Median 4 Monate) eine Pars-plana- bzw. Pars-plicata-Lentekomie durch. Indikation zur Operation war bei allen Kindern eine viususwirksame Linsentrübung (positiver Brückner-Test). Die Operation erfolgte bei allen Säuglingen und Kleinkindern unter Verwendung eines speziell angefertigten 23-Gauge-Aspirations-Irrigations-Systems und eines 23-Gauge-Cutters. Es wurde jeweils eine Linsenaspiration, vordere und hintere Kapsulotomie und vordere Vitrektomie durchgeführt.

Ergebnisse

Unter Verwendung der 23-Gauge-Instrumente konnte bei allen Kindern eine Linsenaspiration, vordere und hintere Kapsulotomie sowie vordere Vitrektomie komplikationslos durchgeführt werden. Das Einsetzen einer Aphakielinse war unmittelbar am Ende der Operation bei allen Kindern problemlos möglich. Der postoperative Verlauf war bei allen Kindern komplikationslos, ein vermehrter Reizzustand bzw. eine Fibrinreaktion wurden nicht beobachtet. Im Nachbeobachtungszeitraum von 2–18 Monaten bildete sich auf einem Auge Nachstar.

Schlussfolgerungen

Die Verwendung von ausschließlich 23-Gauge-Instrumenten im Vergleich zu bisher verwendeten Standard-20-Gauge-Instrumenten ermöglicht ein minimal-invasives Vorgehen bei der Durchführung einer Pars-plana/plicata-Lentektomie unter Erhalt optimaler Operationsergebnisse.

Abstract

Background

Congenital cataract surgery can be performed using a pars plana/plicata approach if placement of an intraocular lens at the time of cataract removal is not a consideration. To prevent development of secondary cataract, we performed lens aspiration, anterior and posterior capsulotomy, and anterior vitrectomy via pars plana or pars plicata approach. Lentectomy in babies should be performed using a minimally invasive technique to prevent complications. Immediately after surgery, insertion of a contact lens should be possible.

Method

The series consisted of 11 eyes of 8 consecutive children who suffered from congenital cataract and underwent pars plana/plicata lentectomy between April 2007 and September 2008. Age at operation ranged from 6 weeks to 13 months (median age 4 months). The inclusion criterion was congenital cataract affecting the visual axis. Lentectomy was performed via a pars plana or pars plicata approach using a special 23-gauge aspiration and irrigation system and a 23-gauge cutter. We performed lens aspiration, anterior and posterior capsulotomy, and anterior vitrectomy.

Results

Postoperatively the eyes showed only mild signs of inflammatory reaction. At the end of the operation, all eyes were provided with contact lenses. For all eyes, the surgical technique provided a clear visual axis during the follow-up period of 2–18 months. One eye developed secondary cataract.

Conclusion

The use of 23-gauge instruments has advantages over standard 20-gauge equipment. Pars plana/plicata lentectomy using 23-gauge instruments is a safe, effective, and minimally invasive method for treating cataract in babies.
