Zusammenfassung
Ziel
Ziel der Untersuchung waren Rückblick und Analyse ultrasonographischer Befunde von Patienten mit nach Kataraktoperationen aufgetretener Endophthalmitis.
Methode
Untersucht wurden die Ultraschallbefunde von 81 Patienten mit Endophthalmitis nach erfolgter Kataraktoperation in einem Zeitraum von 6 Jahren. Des Weiteren wurden Operationsmethode, Zeit des Auftretens der Endophthalmitis und besondere ultrasonographische Befunde untersucht.
Ergebnisse
Im untersuchten Zeitraum entwickelte sich bei 41 Augen eine akute und bei 25 Augen eine subakute Endophthalmitis. Eine spät aufgetretene Endophthalmitis wurde bei 20 Augen festgestellt. Eine postoperative Endophthalmitis trat in 51% der Fälle nach der Phakoemulsifikationsmethode auf. Folgendes wurde mittels Ultraschall festgestellt: Membranbildung in 23 Augen (28%), dichte Trübungen im Glaskörper in 9 Augen und die Verdickung der hinteren Wand in 73 Augen. Zwei Befunde waren häufiger bei akuter und subakuter Endophthalmitis: dichte Glaskörpertrübungen und Abhebung der hinteren Glaskörpermembran.
Schlussfolgerung
Die Erhebung von Ultraschallbefunden am Auge, wie Verdickung der hinteren Wand und verdichtete Organisationen von Trübungen im Glaskörper, kann bei der klinischen Auswertung, Prognose und Behandlung der postoperativen Endophthalmitis sinnvoll sein.
Abstract
Purpose
Review and analysis of ultrasonographic data of patients with endophthalmitis following cataract surgery.
Patients and methods
We conducted a retrospective analysis of data and ultrasound findings of 81 patients with endophthalmitis following cataract surgery between 2000 and 2005. We evaluated the type of surgery, time of onset of endophthalmitis, and different ultrasonographic findings.
Results
During the study period, acute endophthalmitis following cataract surgery developed in 41 eyes, subacute endophthalmitis in 25 eyes, and late-developing endophthalmitis in 20 eyes. In 51% of the cases, the endophthalmitis occurred after the phacoemulsification method. Ultrasonographic findings such as membrane formation were found in 23 eyes (28%), and dense vitreous opacities were detected in nine eyes. Posterior wall thickness was measured in 73 eyes. Two initial echography findings were associated with acute and subacute endophthalmitis: dense vitreous opacities and detachment of the posterior vitreous limiting membrane.
Conclusion
Ultrasonographic findings such as increased thickness of the posterior wall and dense organization of the vitreous can be useful in the clinical evaluation, prediction, and treatment of postoperative endophthalmitis.
