Plötzlicher Visusverlust eines 7-jährigen Patienten

Zusammenfassung

Ein 7-jähriger Junge wurde uns aufgrund einer plötzlichen Visusverminderung beidseits vorgestellt. Im Alter von 4 Monaten wurde ein kongenitales Glaukom beidseits diagnostiziert und eine Trabekulotomie durchgeführt. Es erfolgten regelmäßige Augendruck- und Funduskontrollen. Sowohl in der Magnetresonanztomographie als auch computertomographisch zeigte sich ein supraklinoidal von der rechten A. carotis interna ausgehendes Riesenaneurysma. Nach eingehenden Untersuchungen wurde eine endovaskuläre Ballonokklusion der A. carotis interna mit einem extrakraniell-intrakraniellem Bypass zur Umgehung der insuffizienten kolateralen Zirkulation durchgeführt.

Abstract

A 7-year-old boy presented to us with a sudden bilateral decrease in visual acuity. At the age of 4 months bilateral congenital glaucoma had been diagnosed and trabeculotomy performed. Eye pressure and fundus were monitored regularly. Both magnetic resonance imaging and computed tomography revealed a giant supraclinoid aneurysm originating in the right internal carotid artery. After comprehensive examinations endovascular balloon occlusion of the internal carotid artery was performed in conjunction with extracranial-intracranial bypass to augment the insufficient circulation.
