Zusammenfassung
Hintergrund
Die wellenfrontgeführte Hornhautchirurgie erfordert präoperativ die Wellenfrontanalyse bei dilatierter Pupille. Ein Einfluss von Mydriatika auf monochromatische Aberrationen hätte ein verfälschtes Ablationsprofil zur Folge.
Methoden
Um den Einfluss von Tropicamid und Phenylephrin von dem der reinen Blendenerweiterung zu trennen, wurden Wellenfrontmessungen nach Dunkeladaptation und nach Applikation der Mydriatika nacheinander an jeweils 20 bzw. 19 Augen durchgeführt.
Ergebnisse
Phenylephrin hat bei den untersuchten Pupillendurchmessern keinen signifikanten Einfluss auf die Wellenfront, weder Sphäre noch RMS unterscheiden sich von den Werten der Dunkeladaptation. Nach Applikation von Tropicamid finden sich signifikante Veränderungen des Koeffizienten Z20 von durchschnittlich 0,47 µm. Parallel zeigen sich ebenfalls signifikante Abweichungen der Sphäre von −0,25 dpt. Die Gesamtaberration (RMS) weist hingegen keine Unterschiede auf, lediglich die sphärische Aberration Z420 ist im Mittel signifikant um 0,035 µm verringert.
Schlussfolgerungen
Die medikamentöse Mydriasis durch Phenylephrin und Tropicamid verändert individuell, bei Tropicamid auch statistisch signifikant, die Wellenfront. Es empfiehlt sich daher, bei Messungen vor refraktiven Eingriffen auf beide Mydriatika zu verzichten und nach Möglichkeit in physiologischer Mydriasis zu messen. Sollte durch Abdunklung keine ausreichende Mydriasis erreicht werden, bietet sich Phenylephrin als Mydriatikum der ersten Wahl an. Der Einfluss auf die Wellenfront ist hier am geringsten.
Abstract
Background]
For wave-front guided corneal surgery, measuring higher order monochromatic aberrations in mydriasis is needed. However, a potential influence of mydriatic drugs on such aberrations could distort the ablation profile.
Method
Wave-front analysis was carried out on 20 (tropicamide) and 19 (phenylephrine) eyes after dark adaptation, followed by measurement after the instillation of the mydriatics one after another.
Results
Phenylephrine had no significant influence on the wave-front; neither sphere nor RMS data differed from those taken after dark adaptation. After instilling tropicamide, significant changes in Z20 and, in parallel, also of the sphere were found. The RMS showed no significant difference, only the spherical aberration Z40 was reduced by an average of 0.035 µm.
Conclusion
The wave-front changes individually through the mydriasis due to phenylephrine and tropicamide. In the case of tropicamide, the deviation is statistically significant. Therefore, abandonment of these mydriatics before refractive surgery can be recommended, as can the use physiological pupil dilatation. Because of its lower influence, phenylephrine should be the first choice if dimout effects no adequate mydriasis.
