Zusammenfassung
Ziel
Ziel dieser Studie war die Evaluierung der Effektivität von alleiniger lokaler Steroidtherapie nach perforierender Normalrisikokeratoplastik.
Patienten und Methoden
In einer prospektiven randomisierten klinischen Studie wurden 40 Patienten untersucht, die sich einer perforierenden Normalrisikokeratoplastik unterzogen hatten. Von ihnen wurden 20 postoperativ nur mit Prednisolonacetat-1%-AT zu Beginn 5-mal täglich ausschleichend für 6 Monate behandelt, und 20 Patienten erhielten zusätzlich systemisch Fluocortolon 1 mg/kg Körpergewicht pro Tag ausschleichend über 3 Wochen. Sie wurden hinsichtlich eines klaren Transplantatüberlebens, der Anzahl der Immunreaktionen und der Nebenwirkungen untersucht.
Ergebnisse
Bei einer mittleren postoperativen Nachbeobachtungszeit von 18±9 Monaten wurden 3 endotheliale Immunreaktionen in der Patientengruppe, die eine alleinige lokale Steroidtherapie erhalten hatte, beobachtet. In der Gruppe, die eine kombinierte lokale und systemische Steroidtherapie erhalten hatte, gab es in diesem Zeitraum 2 endotheliale Immunreaktionen. Keiner der Patienten entwickelte bis zum heutigen Zeitpunkt ein irreversibles Transplantatversagen.
Schlussfolgerungen
Eine alleinige passagere lokale Steroidgabe scheint auch ohne zusätzliche systemische Steroidbehandlung eine effektive Immunprophylaxe nach perforierender Normalrisikokeratoplastik darzustellen.
Abstract
Background
The aim of this study was to evaluate the efficacy of sole application of topical steroids after normal-risk keratoplasty.
Patients and methods
This randomized prospective clinical study assessed 40 patients who had undergone penetrating normal-risk keratoplasty. Twenty patients were treated exclusively with prednisolone acetate 1% eye drops 5×/day for 6 months postoperatively. Another 20 patients additionally received systemic fluocortolone 1 mg/kg body weight per day tapered within 3 weeks postoperatively. The main outcome measures included clear graft survival, ratio of graft rejection, and side effects.
Results
The mean postoperative follow-up was 18±9 months. Three graft rejections were observed in the group receiving only topical steroids. Two graft rejections were observed in the group administered combined systemic and topical steroid therapy. None of the patients has developed irreversible graft failure so far.
Conclusion
Sole topical steroid application seems to be an effective immune prophylaxis in patients undergoing penetrating normal-risk keratoplasty.
