Zusammenfassung
Die neurotrophe Keratopathie ist eine degenerative Hornhauterkrankung in Folge einer Beeinträchtigung der kornealen Innervation. Dabei kann der die Hornhaut sensibel versorgende erste Ast des N. trigeminus (N. ophthalmicus) auf seiner gesamten Länge vom Kern im Hirnstamm (z. B. durch Tumoren) bis zum Verlauf in der Hornhaut (z. B. durch refraktive Chirurgie) beschädigt werden. Der Verlust der sensiblen Hornhautinnervation führt zum Einen zu einer Verminderung der Tränensekretion (Sicca-Symptomatik) sowie zum Anderen zu einer Minderversorgung der Hornhaut mit trophischen Faktoren, was wiederum die Regenerationsfähigkeit des Hornhautepithels beeinträchtigt. Der mit reduzierter Oberflächenheilungspotenz kombinierte Tränenmangel führt dann zu persistierenden Epitheldefekten mit Gefahr der Hornhauteinschmelzung und Perforation. Eine frühe Diagnose und adäquate Therapie können die katastrophalen Folgen einer neurotrophen Keratopathie verhindern.
Abstract
Neurotrophic keratitis is a degenerative disease of the cornea caused by reduced corneal innervation. Trauma, tumors, inflammatory lesions and surgical procedures can damage the first branch of the trigeminal nerve on its entire course from brainstem to and within the cornea. Loss or reduction of corneal innervation leads to a reduced aqueous phase of the tear film and due to reduced supply with neurotransmitters/trophic factors also to reduced epithelial healing capacity (impaired mitosis and migration). Combined existence of tear film deficiency and impaired epithelial healing capacity predispose to persistent epithelial defects, corneal ulcers and perforation. Early diagnosis and adequate treatment may prevent this catastrophic chain of events.
