Zusammenfassung
Hintergrund und Ziel. Bei Patienten mit einseitigen peripheren vestibulären Störungen besteht oft eine Abweichung der visuellen subjektiven Vertikalen (VSV).Diese Abweichung ist flüchtig und schnell kompensierbar. Ziel der Studie war zu klären, inwieweit die Abweichung der VSV mit der Zykloduktionsstellung (ZDS) übereinstimmt.
Patienten und Methoden. Prospektiv wurden 20 Patienten mit akutem einseitigen Vestibularisschaden (10 weiblich, 10 männlich, Alter zwischen 16 und 78 Jahren) einbezogen. Einschlusskriterium war eine Abweichung der VSV >2° binokular und/oder eine Differenz der monokular gemessenen VSV von mehr als 1,5°. Zunächst wurde die VSV im Dunkelraum mit Leuchtstab monokular und binokular bestimmt, dann wurde nach Pupillenerweiterung eine Fundusfotografie durchgeführt und manuell der Verrollungswinkel der Achse Papille/Makula gemessen.
Ergebnisse. Sowohl die monokulare VSV (durchschnittlich 7,9°) als auch die ZDS (durchschnittlich 10,6°) waren zur erkrankten Seite abgewichen. Es fand sich eine gute qualitative Korrelation zwischen der VSV und der objektiv gemessenen ZDS am Augenhintergrund. Quantitativ war die ZDS stärker ausgeprägt als die monokulare VSV.
Schlussfolgerung. Im Gegensatz zur bi-nokularen Bestimmung der VSV ergibt sich bei monokularer Bestimmung eine gute Übereinstimmung zur mit der Fundusfotografie gemessenen ZDS. Offen ist noch, welcher Teil des Vestibularapparates die pathologische Zykloversion verursacht. Aufgrund unserer Ergebnisse ist für die klinische Diagnostik die monokulare Bestimmung der VSV zu fordern.
Abstract
Background. A deviation of the subjective visual vertical (svv) is often found in patients with unilateral peripheral vestibular lesions and in most cases the deviation is transitory and easy to compensate. The purpose of the study was to find out if there is a correlation between deviation of the svv and objective cyclodeviation of the retina.
Patients and methods. A total of 20 patients (10 females and 10 males aged between 16 and 78 years) with acute vestibular disease were enrolled. Only patients with a binocular deviation of the svv of more than 2° and/or a difference between the monocular deviation of more than 1.5° were included. The svv was examined monocular in a darkened room and after pupil dilatation, fundus photography was performed on each eye. The angle between papilla and macula was measured manually.
Results. We found a good qualitative correlation between svv (mean 7.9°) and objective cyclodeviation (mean 10.6°).
Conclusion. In contrast to the binocular measurement of the svv, we found good correlation between the monocular svv and the objective cyclorotation measured with fundus photography. It is not yet clear which part of the vestibular system is responsible for cyclodeviation. Because of our results we recommend monocular measurement of the svv in clinical examinations.
