Zusammenfassung
„Functional restoration“ (FR) beschreibt ein Therapieprinzip bei chronischen Schmerzen, das eine Verbesserung der subjektiven und objektiven Funktionsfähigkeit auf somatischer und psychosozialer Ebene anstrebt. Dieses Therapieprinzip, das auf einem biopsychosozialen Schmerzmodell und einem ganzheitlichen, interdisziplinären Ansatz der Behandlung basiert, ist inzwischen Grundlage für weitgehend alle interdisziplinär arbeitenden Schmerztherapieprogramme. Die Diagnostik und Therapie erfolgen interdisziplinär durch ein festes Behandlungsteam mit räumlicher, zeitlicher und konzeptioneller Integration. Strukturelle Pathologien, somatische Funktionsstörungen, psychosoziale Dysfunktionen sowie medizinische und psychologisch-psychiatrische Komorbiditäten sind zu erfassen und in ihrer Bedeutung für die Schmerzproblematik zu bewerten. Aus der Gesamtbetrachtung ergibt sich der individuelle, konzeptionell einheitliche Therapieplan des Patienten. Neben einer Verbesserung der somatischen Leistungsfähigkeit ist für einen nachhaltigen Therapieerfolg die Veränderung dysfunktionaler Muster auf der psychosozialen Ebene erforderlich. In allen Versorgungssektoren, bei unterschiedlichen Schmerzerkrankungen, bei Kindern und Jugendlichen sowie bei Senioren ist FR nach den beschriebenen Prinzipien langfristig wirksam.
Abstract
Functional restoration (FR) describes a therapeutic principle for addressing chronic pain that aims to improve subjective and objective functioning at the somatic and psychosocial levels. The principle, which is based on a biopsychosocial pain model and a holistic, interdisciplinary approach to treatment, is now the basis for practically all interdisciplinary pain therapy programs. Diagnosis and treatment are provided on an interdisciplinary basis by a permanent team with spatial, temporal, and conceptual integration. Structural pathologies, somatic dysfunction, psychosocial dysfunction, and medical and psychological/psychiatric comorbidities are to be recorded and evaluated in terms of their significance for the pain problem. The patient’s individual, conceptually uniform treatment plan results from this overall consideration. In addition to an improvement in somatic performance, a change in dysfunctional patterns at the psychosocial level is necessary for sustainable therapeutic success. In all care sectors, FR according to the described principles is effective in the long term for different pain disorders—in children and adolescents as well as in geriatric patients.
