Zusammenfassung
Beschrieben wird der Fall eines 9‑jährigen Jungen, der im Alter von 2 Monaten eine okzipitale Schädelprellung nach Sturz vom Wickeltisch erlitten hatte und nun aufgrund von Migräne, Schlafstörungen, Ängsten und Autoaggression vorgestellt wurde. Wegen des Verdachts auf zerebrale Krampfanfälle wurde eine Elektroenzephalographie (EEG) durchgeführt, die einen rechtsseitigen intermittierenden, okzipitotemporalen Herdbefund mit verlangsamtem Grundrhythmus zeigte. Osteopathisch-manualmedizinisch wurden ein „locked cranium“ mit somatischen Dysfunktionen in der Synchondrosis sphenobasilaris und in der rechtsseitigen Sutura occipitomastoidea diagnostiziert und behandelt, außerdem wurden Atlasimpulse nach Arlen gesetzt. Im weiteren Verlauf war die EEG-Verlangsamung nicht mehr nachweisbar und die Symptomatik bildete sich zurück. Eine Korrelation zwischen einer suturalen Störung des Schädels und einer funktionellen Störung der Potenzialdifferenz an der Gehirnoberfläche ist in der Literatur bisher nicht beschrieben worden. Angesichts vigilanzabhängiger Schwankungen und funktioneller Variabilität von EEG-Befunden sowie weiterer möglicher Einflussfaktoren lässt diese Beobachtung einen Zusammenhang nur vermuten, einen Beweis kann sie nicht liefern. Denkbare therapeutische Wirkmechanismen auf gestörte Potenzialdifferenzen an der Hirnoberfläche könnten neben der Reduktion der Duraspannung auch eine Verbesserung von Perfusion, Diffusion, lokalem Stoffwechsel und autonomer Regulation sein. Diese Hypothesen bedürfen weiterer Forschung.
Abstract
This article describes the case of a 9-year old boy that slipped from a changing table onto his occiput at the age of 2 months. Due to migraine, sleep disorders, anxiety and autoaggression an electroencephalography (EEG) was performed to exclude cerebral seizures. This showed an intermittent slow-wave focus in the occipito-temporal region. During osteopathic manual therapy a somatic dysfunction presenting as “locked cranium” was found and treated in the ipsilateral occipito-mastoideal suture and in the spheno-basilar synchondrosis. Following osteopathic manual therapy and manipulation of the atlas the slow-wave focus was no longer seen and the symptoms resolved. There are no reports of an effect of sutural dysfunction of the scull on measurable potential differences of the brain causing a slow-wave focus on EEG. Due to functional variability of the EEG and other influencing factors this case hypothesizes, but cannot prove, a correlation. A possible therapeutic effect on the brain’s electrical field by improving dural tension, perfusion, diffusion, local brain metabolism and autonomous regulation is discussed as a potential mode of action. This needs further scientific workup.
