Hintergrund
Beschwerden und Krankheiten des Stütz- und Bewegungsapparates zählen zu den häufigsten Ursachen für Arztkonsultationen und Arbeitsunfähigkeit und verursachen erhebliche Kosten im Gesundheitssystem. Verlässliche epidemiologische Daten über die Häufigkeit von Beschwerden und pathologischen Veränderungen im Bereich des muskuloskeletalen Systems bei Kindern und Jugendlichen sind rar.
Fragestellung
Anliegen der vorliegenden Untersuchung war es, eine regional begrenzte Adoleszentenkohorte bezüglich der Prävalenz von Hüftbeschwerden zu untersuchen und gleichzeitig mögliche Faktoren des sozialen Umfeldes und der Lebensgewohnheiten zu ermitteln, die mit dem Auftreten der Beschwerden assoziiert sind.
Material und Methode
Die Untersuchung wurde im Rahmen der obligatorischen schulärztlichen Untersuchung der Klassenstufe 9 durchgeführt. Die Teilnahme an der Untersuchung war für die Jugendlichen freiwillig. Die Auswertung der Daten erfolgte anonym. Von 2401 Schülern standen 2368 für die Untersuchung und Datenauswertung zur Verfügung. Anhand eines Fragebogen machten sie Angaben zu Alter, Geschlecht, Wohnort, zu Schulart, sportlichen Aktivitäten, Gesundheitsgefühl, emotionaler Stimmung sowie zu Beschwerden der Hüfte. Des Weiteren erfolgten eine Befragung nach ihrem Alkohol- und Nikotinkonsum sowie die klinische Untersuchung durch die Schulärzte mit Erfassung der Körpermaße und Dokumentation pathologischer Befunde bzw. Normabweichungen der Hüfte.
Ergebnisse und Diskussion
Die Prävalenzraten von Hüftschmerzen und -pathologien wurden sowohl in Bezug auf die Gesamtzahl der untersuchten Jugendlichen als auch in Abhängigkeit der genannten Faktoren dargestellt. Hüftschmerzen wurden von 6,4% der Jugendlichen angegeben, bei 2,7% fanden sich pathologische Hüftbefunde. Mädchen litten signifikant häufiger unter Hüftschmerzen als Jungen. Eine Ursache dafür könnte sein, dass psychische Faktoren mit für diese Geschlechtspräferenz verantwortlich sind. Bei Gymnasiasten zeigte sich eine signifikant höhere Prävalenz an Hüftschmerzen gegenüber den Schülern anderer Schulformen. Nikotin- und Alkoholgenuss bei Jugendlichen waren mit einer erhöhten Hüftschmerzprävalenz assoziiert. Rauchen wurde als ein signifikanter Risikoindikator für die Entwicklung von Hüftschmerzen gefunden. Mitverantwortlich dafür könnten eine erhöhte Schmerzempfindlichkeit und häufig bestehende psychische Auffälligkeiten bei Rauchern sein.
Abstract
Background
Complaints and disorders of the support and musculoskeletal system are among the most frequent causes for physician consultations and work disability and generate considerable costs for the health care system. Reliable epidemiological data on the incidence of complaints and pathological changes in the musculoskeletal system in children and adolescents are rare.
Aim
The purpose of this study was to examine a cohort of adolescents from a limited geographical area in terms of the prevalence of hip complaints and at the same time identify factors of the social environment and lifestyle habits which are associated with the appearance of the complaints.
Subjects and methods
The study was performed as part of the compulsory school medical check-up of ninth graders. The adolescents’ participation in the study was voluntary. Anonymous data analysis was conducted. Of the 2,401 pupils in ninth grade, 2,368 were available for the study. The pupils entered information in a questionnaire on age, gender, place of residence, type of school, sports activities, health feelings, emotional state, and hip complaints. They were also questioned regarding alcohol and nicotine consumption, and the clinical examination was carried out by the school physician who noted body measurements and documented pathological findings or any divergence from the norm in the hips.
Results and discussion
The prevalence rates of hip pain and pathological findings were determined in relation to the total number of adolescents examined as well as depending on the above-mentioned factors. Hip pain was reported by 6.4% of the adolescents and pathological findings in the hip were detected in 2.7%. Girls suffered significantly more often from hip pain than boys. One cause could be that psychological factors are co-responsible for this gender preference. High school pupils exhibited a significantly higher prevalence of hip pain than pupils attending other types of school. Nicotine and alcohol consumption by adolescents was associated with an increased prevalence of hip pain. Smoking was found to be a significant risk indicator for the development of hip pain. Increased pain sensitivity and frequent presence of distinctive psychological features observed in smokers could be jointly responsible for these results.
