Zusammenfassung
Hintergrund
Die so genannte hohe Luxation der Hüftgelenke zählt zu den typischen Komplikationen der infantilen Zerebralparese (ICP) im Kindesalter. Sie führt oftmals zu zusätzlichen schweren funktionellen Einschränkungen und erhöhter Schmerzhaftigkeit. Über die operative Behandlung hoher Hüftluxationen bei ICP wird kontrovers diskutiert.
Patienten und Methoden
Die vorliegende Studie zeigt unsere Ergebnisse der operativen Behandlung derartiger Patienten im Zeitraum 1969–1997 bzw. 1990–1999. Insgesamt wurden 44 Patienten mit einer Kopfhalsresektion bzw. einer Angulationsosteotomie (Gruppe 1) und 96 Patienten mit einer rekonstruktiven Operation (Becken- und/oder Femurosteotomie, Gruppe 2) behandelt.
Ergebnisse
In der Gruppe 1 wurden 74 vs. 80% gute klinische Resultate und in 66% der Fälle eine deutliche Schmerzreduktion erreicht. Postoperativ eine Funktionsverschlechterung erlitten 20% der Patienten. In der Gruppe 2 konnte die Transferfähigkeit in 43% der Fälle erreicht werden. Bei 14% der Patienten wurde postoperativ freies Gehen möglich. Komplikationen sahen wir bei 8 Patienten (8%).
Abstract
Background
The so-called high hip dislocation is one of the typical complications of infantile cerebral palsy (ICP) in childhood. Due to the luxation the global functional ability of the hip is decreased. However, these patients suffer from severe pain of the hip joint. The operative treatment of hip dislocation in cases of ICP is controversially discussed.
Patients and methods
This study aimed to evaluate the clinical outcome of different operative procedures in cases of hip dislocation and ICP. A total of 111 patients with ICP and hip dislocation were treated operatively in our institution from 1967 to 1999. Forty-four patients underwent resection of the femoral head or angulation osteotomy of the femur (group 1). Ninety-six patients were treated by reconstructive surgery (osteotomy of the pelvis or the femur, group 2).
Results
Group 1: 74 vs 80% of the patients showed good clinical results; 66% of the patients had a reduction in pain perception. Nine patients (20%) suffered from a complication. Group 2: 43% of the patients were able to be transferred postoperatively and 14% could walk without any help. Complications were observed in eight patients (8%).
