Zusammenfassung
Die Okklusion im Milch- und permanenten Gebiss dominiert durch das Höcker- und Fissurenrelief die Stellung der Mandibula zum Schädel in der Schlussbissstellung (maximale Interkuspidation). Die Muskulatur sorgt für den Kieferschluss, die endgültige Position der Kiefergelenke wird bestimmt durch die Zahnstellung bzw. die Okklusion und nimmt damit unmittelbar Einfluss auf den dorsalen und ventralen muskulären oberen Stütz- und Bewegungsapparat.
Fehlstellungen der Mandibula können eine Ursache für Probleme im Stütz- und Bewegungsapparat sein, vice versa können Pathologien im Stütz- und Bewegungsapparat Fehlstellungen der Mandibula verursachen.
Diese strukturellen Fehler sollten frühzeitig erkannt und behandelt werden. Unter anderem aus diesem Grund hat der Berufsverband der Kieferorthopäden mit dem Berufsverband der Ärzte für Kinderheilkunde und Jugendmedizin mit Unterstützung und Beratung durch das Ärzteseminar Hamm-Boppard der Deutschen Gesellschaft für Manuelle Medizin und der wissenschaftlichen Umsetzung durch die Westfälische Wilhelmsuniversität Münster, Poliklinik für Kieferorthopädie einen Leitfaden zur kinderärztlich-kieferorthopädischen Untersuchung entwickelt.
Abstract
In both the first and the permanent dentition, the final occlusion (maximal intercuspation) is predominant in determining the position of the mandible relative to the skull, by way of the profile of cusps and fossae. The musculature brings about the closure of the jaw, and the final position of the temporomandibular joints is determind by the position of the teeth in the upper and lower jaws relative to each other, i.e. the occlusion, thus directly influencing the upper dorsal and ventral muscular part of the musculoskeletal system.
Malposition of the mandibles can give rise to of problems in the musculoskeletal system, and conversely pathologic conditions of the musculoskeletal system can cause malposition of the jaws. This is one reason why the professional association for orthodontists and that for specialists in paediatric and adolescent medicine have jointly elaborated a guideline for paediatric / orthodontic examination with support from and in consultation with the Hamm-Boppard doctors' section of the German Society for Manual Medicine, with scientific implementation by the Orthodontics Polyclinic at the Westphalian Wilhelm University in Munster.
