Zusammenfassung
Dermatofibrome sind eine häufige, benigne, fibrohistiozytär reaktive Läsion, die sich in einer Vielzahl von klinisch-pathologischen Varianten präsentiert. Dabei kommt es teilweise zu erheblichen Abgrenzungschwierigkeiten gegenüber verschiedensten benignen und malignen Tumoren. Nach ihren Besonderheiten werden unterschieden: 1. Dermatofibrome mit architektonischen Besonderheiten wie tief eindringendes Dermatofibrom, Riesendermatofibrom, atrophes Dermatofibrom, aneurysmatisches („angiomatoides”) fibröses Histiozytom, hämangioperizytomähnliches fibröses Histiozytom, Dermatofibrom mit Palisadenformation und ossifizierendes Dermatofibrom. 2. Dermatofibrome mit zellulären/stromalen Besonderheiten wie Klarzelldermatofibrom, Granularzelldermatofibrom, myofibroblastäres Dermatofibrom, sklerotisches Dermatofibrom, Dermatofibrom mit Monsterzellen, atypisches („pseudosarkomatöses”) fibröses Histiozytom, elusives („hämosiderrhotisches”) Dermatofibrom, cholesterotisches Dermatofibrom, myxoides Dermatofibrom.
3. Dermatofibrome mit architektonischen und zellulären/stromalen Besonderheiten in homogenem Muster wie Epithelioidzellhistiozytom, zellulär benignes fibröses Histiozytom, Dermatofibrome mit Proliferation von glatter Muskulatur, basaliomimitierendes Dermatofibrom, pseudolymphomatöses Dermatofibrom, Riesenzellangiohistiozytom, zelluläres Neurothekeom, plexiform fibrohistiozytärer Tumor, plexiformes Xanthom, plexiform xanthomatöser Tumor. 4. Komplexe oder zusammengesetzte Dermatofibrome, bei denen 2 oder mehrere architektonische und zelluläre/stromale Besonderheiten inhomogen vermengt sind, z.B. Silhouette eines Epithelioidzellhistiozytoms mit plexiformem Aufbau eines Neurothekeoms sowie Granularzellveränderungen.
Summary
Dermatofibroma is a common benign fibrohistiocytic lesion which presents with a wide variety of clinicopathologic variants. This may cause great difficulties in delineation from a variety of benign and malignant tumours. According to their peculiarities we differentiate: 1. Dermatofibromas with architectural peculiarities like deep penetrating, atrophic, giant, aneurysmal („angiomatoid”), haemangiopericytoma-like, palisading or ossifying variants. 2. Dermatofibromas with cellular/stromal peculiarities like clear cell, granular cell, myofibroblastic, sclerotic, monster cell, atypical („pseudosarcomatous”), elusive („haemosiderotic”), cholesterotic, and myxoid variants. 3. Dermatofibromas with architectural and cellular/stromal peculiarities in homogenous arrangement like epithelioid cell, cellular benign variants, with smooth muscle proliferation, basal cell carcinoma-like, pseudolymphomatous, multinucleate cell angiohistiocytoma, cellular neurothekeoma, plexiform fibrohistiocytic tumour, plexiform xanthoma and plexiform xanthomatous tumour. 4. Complex or composite dermatofibromas with two or more architectural and cellular/stromal peculiarities in inhomogenous arrangement, e.g. silhouette of an epithelioid cell histiocytoma with plexiform fascicles of cellular neurothekeoma and granular cell features.
