Zusammenfassung
Vorgestellt wird die Pathologie der Rhinoskleroms am Beispiel eines 38 Jahre alten Mannes in Verbindung mit klinischen Befunden. Dieser im bioptischen Eingangsgut selten beobachtete, klinisch tumorartig imponierende Prozeß weist ein charakteristisches plasmazellreiches entzündliches Infiltrat auf, das rasenförmige Schaumzellnester (sog. Mikulicz-Zellen) einschließt, in denen ultrastrukturell der Erreger (Klebsiella rhinoscleromatis) nachgewiesen werden kann, der die Diagnose morphologisch allein sichert. Eine Lokalbehandlung mit 2%iger Acriflavin-Lösung führte zu einer deutlichen klinischen Remission.
Summary
Histologic, immunohistochemical and ultrastructural results of a rhinoscleroma diagnosed bioptically on a 38 aged man are demonstrated. The characteristic light microscopic sign is a plasmocytic inflammatory infiltrate with dense packed foamy transformed macrophages (Mikulicz cells) ultrastructurally containing diplobacilli (Klebsiella rhinoscleromatis, gram negative agent of the lesion). The lesion has to be differentiated as of other inflammatory diseases as of plasmocytoma. A local application of acriflavin solution 2% was of therapeutical benefit.
