Neuroendokrine Tumoren des Magens Pathogenese, Klassifikation und Klinik*

Zusammenfassung

Die neuroendokrinen Tumoren des Magens werden morphologisch in vier Gruppen eingeteilt: 1. Karzinoid-Typ I bei atrophischer Korpusgastritis, 2. Karzinoid-Typ II bei multipler endokriner Neoplasie Typ 1, 3. Karzinoid-Typ III, sporadisch sowie 4. neuroendokrines Karzinom.

Die beiden erstgenannten Formen entwickeln sich in einer histologisch faßbaren Sequenz nach dem Muster Hyperplasie – Dysplasie – Neoplasie. Ihre Pathogenese ist abhängig von einem Zusammenspiel begünstigender Faktoren wie vor allem der Hypergastrinämie und transformierender Faktoren, deren Rolle zur Zeit noch weiter untersucht wird. Im Gegensatz zu den beiden anderen Tumortypen mit ungünstigem Verhalten ist ihre Prognose grundsätzlich günstig. Pathogenese und Vorläuferläsionen der beiden letztgenannten Tumortypen sind noch ungeklärt.

Neuroendocrine tumors of the stomach are classified into four types mostly on the basis of their pathological associations: 1) carcinoid type I, associated with atrophic corporal gastritis, 2) carcinoid type II, associated with multiple endocrine neoplasia type 1, 3) carcinoid type III, sporadic, and 4) neuroendocrine carcinoma.

The first two types develop through a histologically recognizable sequence hyperplasia – dysplasia – neoplasia. Their pathogenesis depends on the combined action of promoting agents, mostly hypergastrinemia, and of transforming agents, the role of which is currently under investigation. Their prognosis is generally favourable in sharp contrast with that of the other two types of tumors, which is usually poor. The pathogenesis and the precursor lesions of the latter neoplasms have not been clarified yet.
