Zusammenfassung
Kopfspeicheldrüsen zeichnen sich durch ein mukosaassoziiertes lymphatisches Gewebe aus und sind oft mit intra- und periglandulären Lymphknoten assoziiert. Entzündungen gehen vielfach mit Gangektasie, lymphoepithelialen Läsionen, Atrophie und Fibrose einher. Das Sjögren-Syndrom kann sich an den Speicheldrüsen in Form einer sog. myoepithelialen Sialadenitis manifestieren, die durch lymphoepitheliale Läsionen wesentlich gekennzeichnet ist. Auf dem Boden dieser Erkrankung, aber auch de novo können primäre Lymphome der Speicheldrüsen entstehen, die am häufigsten den Marginalzonenlymphomen, seltener den monozytoiden B-Zell-Lymphomen und anderen Entitäten zuzuordnen sind. In einem erheblichen Teil sind Lymphominfiltrate in den Speicheldrüsen jedoch Ausdruck einer systemischen Erkrankung entweder in Form einer Auftreibung der Speicheldrüsen durch Infiltrate lymphatischer Leukämien oder als lokalisierter Befall durch primär nodale Non-Hodgkin-Lymphome in intra- oder periglandulären Lymphknoten mit Übergriff auf das Drüsenparenchym. Eine Differenzierung der Entitäten gelingt gewöhnlich durch konventionelle Histologie und Immunhistologie.
Abstract
This tutorial focuses on malignant lymphomas and inflammatory conditions as potential precursors of primary malignant lymphomas in salivary glands. Salivary glands display a mucosa-associated lymphoid tissue and are often associated with intra- and periglandular lymph nodes. Inflammation of the glands may variably produce duct ectasia, lymphoepithelial lesions, atrophy, and fibrosis. Primary lymphomas of the salivary glands may develop on the basis of autoimmune sialadenitis and comprise most frequently marginal zone lymphomas, less often monocytoid lymphomas or other lymphoma entities. In a large proportion of cases, lymphoma infiltrates are attributable to extraglandular lymphomas either as a leukemic infiltrate with diffuse enlargement or as a localized manifestation of primary nodal non-Hodgkin lymphomas. Diagnosis is based on conventional histology and immunohistology.
