Zusammenfassung
Angesichts der in Kreisen analytischer Psychotherapeuten aufgebrochenen Diskussion um die Verwendung von ICD-10-Diagnosen in der psychotherapeutischen Praxis wird die z. Z. gebräuchliche Vorgehensweise analysiert. 100 unausgelesene Diagnosen von tiefenpsychologisch fundierten und analytischen Psychotherapeuten werden anhand der inhaltlichen Kategorien beschrieben. Es lassen sich die Schwerpunkte Symptomatik, neurotische Störungsbilder, Persönlichkeit und Struktur, psychodynamische Beschreibung unterscheiden. Dabei zeigt sich, dass diese frei formulierten psychodynamischen Diagnosen in Form und Inhalt sehr variabel gehandhabt werden. Die Diagnosen unterscheiden sich bezüglich der Logik ihres Aufbaus, der verwendeten Begriffe und der Differenziertheit der Darstellung erheblich. Depressionsbegriffe unterschiedlicher Art sind in 80% der Diagnosen enthalten. Die frei formulierten Diagnosen sind, da keine Einigung über die verwendete Begrifflichkeit vorliegt, für jedwede Form der wissenschaftlichen Kommunikation, sei sie im Dienste der Qualitätssicherung oder der Forschung, ungeeignet. Abschließend wird am Beispiel der Verwendung standardisierter Diagnosen nach OPD der Zugewinn an differenzierter Information illustriert.
Abstract
In view of the emerged discussion among analytic psychotherapists about the use of ICD-10-diagnoses in the psychotherapeutic practice, the at present common method is being analysed. 100 unselected diagnoses of psychotherapists who work on the basis of depth psychology and on analytic grounds are being described according to content categories. Symptomatology, neurotic disorder pictures, personality and structure, psychodynamic description can be distinguished as main foci. It turns out that these freely formulated psychodynamic diagnoses are used very differently regarding content and form. The diagnoses differ extremely with regard to the structure logic, the used terms and the differentiation of the description. Depression terms of different kind are included in 80% of the diagnoses. The freely formulated diagnoses are not suitable for any form of scientific communication, neither for quality control nor for research purposes, as there exists no conformity in using the terms. Finally, the increase in differentiated information is illustrated by the example of using the standardised diagnoses according to OPD.
