Zusammenfassung
Hintergrund
Der subjektive Tinnitus, unter dem etwa 20–25 % der Bevölkerung Deutschlands und 4 % chronisch leiden, umfasst eine Hörwahrnehmung ohne äußere erkennbare Schallquelle. Zwar empfiehlt die aktuelle Leitlinie zur Tinnitusbehandlung auch eine psychosomatische Diagnostik und ggf. (Mit‑)Behandlung, dennoch gibt es bisher wenige gezielte psychosomatische Therapieansätze für Tinnitus.
Fragestellung
Mit dem Ziel, die subjektive Symptombelastung durch eine Verknüpfung von Tinnitus mit der emotionalen Befindlichkeit und Belastung der Betroffenen zu reduzieren, wurde ein Konzept zur ambulanten gruppenmusiktherapeutischen Intervention entwickelt.
Methode
Die Untersuchung erfolgte als monozentrische, nichtkontrollierte Ein-Gruppen-Therapiestudie im Prä-post-Design. Die Behandlung in einer geschlossenen Gruppe von 7 bis 9 Teilnehmer:innen ist als Kurzzeittherapie über 10 wöchentliche Sitzungen zu 90 min manualisiert. Es sind Gruppenpsycho- und Musiktherapie sowie Entspannung im psychodynamischen Kontext integriert. Neben einer Eingangsdiagnostik wurden die Teilnehmer:innen vor, am Ende und 6 Monate nach der Intervention psychometrisch getestet. Mit dem Tinnitus-Beeinträchtigungs-Fragebogen (TBF-12) und dem Mini-Tinnitusfragebogen (Mini-TF) wurde die spezifische, mithilfe der Brief Symptom Inventory (BSI) die allgemeine Symptombelastung erfasst.
Ergebnisse
Bisher wurden 7 ambulante Gruppenmusiktherapien mit insgesamt 50 Teilnehmer:innen abgeschlossen. Die Symptombelastung durch Tinnitus (Mini-TF) war zu Behandlungsbeginn im Durchschnitt schwergradig, nach Abschluss der Therapie mittelgradig. Eine Verbesserung konnte v. a. im emotional-kognitiven Bereich des TBF-12 nachgewiesen werden. Hingegen kam es bei den allgemeinen psychischen Störungen weder für den Gesamtwert des BSI noch für die Syndromskalen zu einer relevanten Veränderung im Behandlungsverlauf.
Diskussion
Die ersten Befunde zeigen, dass das ambulante gruppenmusiktherapeutische Angebot die subjektive Symptombelastung durch Tinnitus signifikant reduzieren kann. Die Ergebnisse müssen jedoch an einer größeren Stichprobe, im Vergleich zu anderen Behandlungsmethoden und hinsichtlich der Stabilität weiter evaluiert werden.
Abstract
Background
Subjective tinnitus, from which about 20–25% of the population suffer from acute forms and 4% from chronic forms, comprises an auditory perception without an externally recognizable sound source. Although the current guidelines for tinnitus treatment also recommend psychosomatic diagnosis and, if necessary, (co)treatment, to date there have been few targeted psychosomatic therapy approaches for tinnitus.
Objective
With the aim of reducing the subjective symptom burden by linking tinnitus with the emotional state and stress of those affected, a concept for outpatient music group therapy intervention was developed.
Method
The study was conducted as a monocentric, uncontrolled, single-group evaluation study in a pre-post design. Treatment in a closed group of 7–9 participants is manualized as short-term therapy over 10 weekly sessions of 90 min. Group psychotherapy, music therapy, and relaxation are integrated in a psychodynamic context. In addition to an initial diagnostic test, participants were psychometrically tested before, at the end, and 6 months after the intervention. The tinnitus impairment questionnaire (TBF-12) and the mini-tinnitus questionnaire (Mini-TF) were used to assess the specific symptom burden and the brief symptom inventory (BSI) was used to assess the general symptom burden.
Results
To date, 7 outpatient music group therapies have been completed with a total of 50 participants. The symptom burden of tinnitus (Mini-TF) was on average severe at the beginning of treatment and moderate after completion of therapy. An improvement could be demonstrated especially in the emotional-cognitive domain of the TBF-12. In contrast, there was no relevant change in general psychopathology during the course of treatment, neither for the total score of the BSI nor for the syndrome scales.
Conclusion
The initial findings show that the outpatient music group therapy program can significantly reduce the subjective symptom burden of tinnitus; however, the results need to be further evaluated on a larger sample, in comparison to other treatment methods, and with respect to stability.
