Abstract
Psychotherapy as a treatment for sick persons wants to support people to overcome their suffering. In order to be able to achieve this goal effectively and with a high degree of accuracy, guidelines and manuals are increasingly being provided and the actual effectiveness in psychotherapy research is being checked. When a goal in psychotherapy occasionally cannot be achieved, the question of causes arises. A possible explanation is failure. Errors, mistakes and rule violations are others. Distinguishing them can clarify causes and encourage learning from them.
