Internetsucht und „internetbezogene Störungen“

Zusammenfassung

Nach einer Darstellung des aktuellen Forschungsstandes zum Störungsbild Internetsucht und der Abbildung der diagnostischen Entitäten werden zunächst Ursachen und Auswirkungen der Internet- und Computerspielsucht im Kontext von Verhaltenssüchten beschrieben. Dem Suchtverhalten zugrunde liegende Lernmechanismen werden dargestellt und mit dem Störungsbild in Verbindung gebracht. Komorbide Erkrankungen, insbesondere der Einfluss der Depression als Folge oder mögliche Primärerkrankung, werden anhand der aktuellen Forschungsliteratur beschrieben. Der vermutete Einfluss von exzessivem Spielverhalten auf aggressive Einstellungen wird kritisch beleuchtet und in den aktuellen Forschungsstand eingebettet. Die klinische Erfahrung zeigt, dass bei Studierenden Internetsucht häufig mit Prokrastination, sozialen Ängsten und depressiven Verstimmungen verbunden ist. Die Identifikation von auslösenden Gedanken, Gefühlen und Situationen wird als zentrales Element der Behandlung von Computerspiel- und Internetsucht beschrieben. Dabei werden Versatzstücke eines kognitiv-behavioralen Therapiemanuals skizziert. Hierzu zählen u. a. der Motivationsaufbau, eine Zielanalyse, Verhaltensanalysen, Psychoedukation, eine Einbindung des sozialen Umfelds, eine Expositionsbehandlung und weitere unterstützende therapeutische Techniken. Die Ergebnisse zur Wirksamkeit der Behandlungen von Internetsucht werden vorgestellt, und auf die aktuell noch methodisch unzureichende Studienlage wird hingewiesen.

Abstract

Predictors as well as consequences of internet and computer game addiction are delineated in detail as well as the current state of research. Furthermore, diagnostic entities, such as the upcoming behavioral addiction are defined and linked with learning processes. Comorbidities, especially depression are debated in terms of pre-existing illnesses or consequences of behavioral addictions. Aggressive behavior and its shift from online to offline behavior are discussed. Clinical experience shows that students with internet addiction often also show procrastination, social phobia and depressive symptoms. Core elements of a cognitive behavioral therapy are introduced and include: elaboration on motivational state, an analysis of patients’ psychotherapeutic aims, behavioral analysis, psychoeducation, the responsibility and integration of the social environment, exposure therapy and additional therapeutic techniques. Recently, in the field of internet and computer game addiction, only few results of efficacy studies have been published. Additionally, the majority of these results did not fulfill the criteria of good clinical practice. The content of the following manuscript culminates in suggestions for the therapeutic practice.
