Zusammenfassung
In Deutschland sterben pro Jahr etwa 10.000 Menschen an einem Suizid. Zum Zeitpunkt der Selbsttötung leiden 90 % der Betroffenen an einer psychischen Erkrankung. In Ätiologiemodellen wird Variablen wie Hoffnungslosigkeit, kognitiver Einengung, sozialer Isolation, der Wahrnehmung, eine Last zu sein, und verminderter Furcht vor dem Tod eine zentrale Rolle bei der Entstehung suizidaler Intentionen beigemessen. Im Rahmen der Krisenintervention müssen diese Risikomerkmale sorgfältig erhoben werden. Zudem bedarf es einer beziehungsorientierten, ambivalenzfreundlichen Auseinandersetzung mit Suizidabsichten, um eine Entscheidung über das therapeutische Setting treffen zu können. In diesem Beitrag werden Definitionskriterien suizidalen Verhaltens, die Verbreitung suizidbezogener Phänomene und Risikomerkmale für vollendete Suizide beschrieben, psychologische Erklärungsmodelle werden vorgestellt, und es wird auf Strategien der Risikoabschätzung sowie Krisenintervention eingegangen.
Abstract
About 10,000 persons die by suicide in Germany each year and 90 % of these persons suffer from a mental disorder at the time of suicide. In theoretical models on the development of suicidal behavior the importance of psychosocial variables, such as hopelessness, attentional fixation, social isolation, perceived burdensomeness and reduced fear of death and dying is stressed. In the course of crisis intervention, a sorrow risk assessment has to take place and it needs a relationship-oriented, ambivalence-friendly debate on suicidal intentions, before deciding on the correct treatment setting. This article provides a description of the nomenclature of suicidal phenomena as well as an epidemiology of suicides, suicide attempts and suicidal ideation, refers to the etiology of suicidal behavior and points out central principles of risk assessment and crisis intervention in dealing with suicidal patients.
