Abstract
Pectus carinatum is an uncommon malformation that is often more symptomatic than the appearance suggests, and one that physicians often do not refer for surgical correction. Hospital records of 90 patients who underwent repair of pectus carinatum deformities between 1970 and 2000 were reviewed. During the same period another 445 patients underwent repair of excavatum deformities. Minimal deformity was observed before the age of 10 years for 81 of 90 patients; only 7 of the 90 underwent repair before age 11 years. AH patients were symptomatic; 84 had exertional dyspnea and exercise limitation, 52 had frequent respiratory infections, 24 had asthma, and 38 had chest discomfort. The mean pectus severity score (width of chest divided by the distance between the sternum and spine) was 1.73 (the normal chest is 2.56). The type of repair varied with the type of deformity, consisting of subperiosteal resection of the deformed cartilages, transverse osteotomy of the anterior sternum with insertion of a cartilage wedge, and support with a steel strut for 4 to 6 months in 76 of 90. There were no deaths within 1 year after the repair. Complications included hypertrophic scar (n=13), wound seroma (n=5), pleural effusion (n=3), and pneumothorax (n=2). The mean blood loss was 78 ml, and the mean hospital stay was 2.6 days. With a mean follow-up of 12.8 years, all patients experienced alleviation of respiratory symptoms and chest discomfort and diminished exercise limitation; 88 of 90 patients experienced a very good to excellent long-term result. One patient required reoperation. Pectus carinatum often causes more severe respiratory symptoms and exercise limitation than is generally recognized. Repair in 90 patients with carinatum deformities has resulted in marked clinical improvement in all patients, with low morbidity and short hospitalization.

Le pectus carinatum est une malformation peu fréquente mais plus souvent symptomatique que son aspect physique ne suggère: peu de médecins demandent une correction chirurgicale. On a analysé les dossiers de 90 patients ayant eu une opération pour pectus carinatum entre 1970 et 2000. Pendant la même période, on a opéré 445 patients pour un thorax en entonnoir (pectus excavatum). Une déformation minime a été observée avant l’âge de 10 ans pour 81/90 patients; seulement 7/90 ont été corrigés avant l’âge de 11 ans. Tous les patients étaient symptomatiques; 84 avaient une dyspnée d’effort et étaient limités dans l’effort physique, 52 avaient des infections respiratoires répétées, 24 avaient un asthme, et 38 avaient une gène thoracique. Le score de sévérité moyen (largueur du thorax divisé par la distance entre le sternum et le rachis) a été de 1,73 (normal=2,56). Le type de réparation a varié avec le type de malformation, consistant en une résection sous-périostée des cartilages déformés, une ostéotomie transverse du sternum avec insertion d’un coin de cartilage, et une attelle en acier pendant 4 à 6 moins chez 76/90 patients. II n’y a eu aucune mortalité à un an. Les complications étaient une cicatrice hypertrophique (n=13), un sérome de la plaie (n=5), un épanchement pleural (n=3) et un pneumothorax (n=2). Les pertes sanguines moyennes ont été de 78 ml. La durée moyenne de séjour a et e de 2,6 jours. Avec un suivi moyen de 12,8 ans, tous les patients se sont améliorés au plan respiratoire, au plan de la limitation d’effort, et de la gêne thoracique. 88/90 patients ont eu un résultat très bon à excellent. Un seul patient a nécessité une réintervention. Le syndrome de pectus carinatum est responsable de symptômes respiratoires sévères et de limitation d’efforts beaucoup plus fréquents qu’on le pensait autrefois. On a constaté une amélioration clinique remarquable après cure de cette malformation chez 90 patients avec une morbidité basse et une hospitalisation courte.

El “pectus carinatum gallinaceum” o tórax en quilla es una rara malformación, que produce síntomas con más frecuencia de lo que parece y que pocas veces es remitida para su corrección quirúrgica. Se revisan las historias de 90 pacientes que fueron tratados quirúrgicamente de su “pectus carinatum” entre 1970 y 2000. En este periodo se trataron quirúrgicamente 445 pacientes con “pectus excavatum” o tórax en embudo. En 81/90 pacientes se observaron deformidades mínimas antes de los 10 años de edad y sólo 7/90 fueron intervenidos antes de cumplir 11 años. Todos los pacientes presentaban síntomas: 84 disnea de esfuerzo con limitación de su actividad física, 52 cursaron con infecciones respiratorias repetidas, 24 padecían asma y 38 molestias vagas en tórax. El grado medio de la gravedad del “pectus carinatum” (se obtiene dividiendo la anchura del tórax por la distancia entre el esternón y la columna) fue de 1.73 (en un tórax normal es de 2.56). El tratamiento reconstructor fue diferente según el tipo de deformidad que presentaba el enfermo. En 76/90 pacientes se realizaron resecciones subperiósticas de los cartílagos deformados, osteotomías transversas anteriores del esternón con inserción de cuñas de cartílago fijadas mediante riostras de acero durante 4 a 6 meses. No hubo mortalidad alguna al año de la intervención. Las complicaciones fueron: cicatriz queloidea (13 casos), seroma de la herida (5), derrame pleural (3), pneumotórax (2). La hemorragia intraoperatoria media fue de 78 mi. Estancia hospitalaria media 2.6 dias. Tras un seguimiento medio de 12.8 años se constató que todos los pacientes mejoraron de su sintomatologia respiratoria, aumentando su actividad física y disminuyendo las molestias torácicas. En 88/90 se obtuvieron excelentes resultados a largo plazo. Un solo paciente requirió una reintervención. El “pectus carinatum”, como reconocen todos los autores, origina con frecuencia síntomas respiratorios graves, limitando la actividad física de los pacientes. La reconstrucción torácica en 90 pacientes con esta malformación mejoró extraordinariamente la sintomatologia, siendo la morbilidad escasa y la estancia hospitalaria corta.
