Loosening of threaded acetabular cups in arthroplasty of the hip

Summary. Seventy-nine patients underwent hip arthroplasty using a threaded acetabular cup during 1988 and 1989. Out of 71 cups implanted for coxarthrosis, radiographs of 62 have been reviewed at a mean follow up of 69 months. The survival rate at 5.5 years was 77.5%. The mean age at operation was 61 years. Loosening of the cup occurred in 7 cases with pain on walking; 5 had a revision operation. In another 28 patients, radiolucent lines greater than 1 mm were found around the cup and 7 of these were unstable. The previous coxarthrosis was classified as atrophic with 5 loose cups, normotrophic with 2 loose cups and hypertrophic with no loosening. This difference between loosening in the atrophic and hypertrophic types of preexisting coxarthrosis was statistically significant. Radiolucent lines were present in 61% of the atrophic cases, 53.4% of the normotrophic and 25% of the hypertrophic, which was also significant. If the cases of loosening are added to those with radiolucent lines, the types of coxarthrosis show highly significant differences. Threaded cups had a high percentage of failure which was greater in cases with atrophic coxarthrosis.

Résumé. Pendant les années 88 et 89 nous avons opéré 79 patients pour arthroplastie de la hanche avec un cotyle vissé. Des 71 anneaux qui ont été implantés pour une coxarthrose, nous avons analysé le comportement radiologique de 62 qui ont été suivis jusqu’à ce jour. L’âge moyen des patients était 61 ans (47 – 71). La période de contrôle a été de 69 mois en moyenne (61 – 81) et la survie au bout de 5 ans ¹ / ₂

est de 77,5%. Pour 7 cas (11,3%) parmi lesquels 5 ont été réopérés, on a noté une mobilisation cotyloïdienne accompagnée de douleurs et de boiterie. Chez 28 autres patients, on a noté des liserés péri-prothétiques supérieurs à 1 mm dont 7 (11,3%) étaient évolutifs avec une augmentation d’épaisseur sur les radiographies successives. L’arthrose avant traitement a été classée comme atrophique, normo-trophique et hypertrophique. 5 mobilisations ont eu lieu dans les formes atrophiques (21,7%), 2 dans les formes normo-trophiques (13,4%) et aucune dans les hypertrophiques avec une différence significative entre les atrophiques et les hypertrophiques (p = 0,01). La présence de liserés périprothétiques a été constatée dans 61% des formes atrophiques, dans 53,4% des normo-trophiques et dans 25% des hypertrophiques avec une différence également significative. Les anomalies radiologiques des anneaux cotyloïdiens en ajoutant les mobilisations et les liserés ont montré une différence très significative (p = 0,0002) par rapport au type d’arthrose. Les arthroplasties avec cotyle vissé ont un pourcentage élevé d’échecs surtout lorsque l’arthrose est atrophique.
