A new trapezoid lower eyelid clamp to shape the lower conjunctival fornix

Abstract

Eyelid clamps tend to rotate laterally in lower eyelid operations in which the medial eyelid is stabilised in a shallow position, making medial dissections difficult. Thus, this study aimed creating a lower eyelid clamp that can avoid intraoperative lateral rotations. In addition, the potential shape of the lower conjunctival fornix was achieved from the contour of the new clamp. The clamp was designed by simulating the eyelid area obliquely covered by the usual eyelid clamps. The medial and lateral lengths of the clamp were 10 and 16 mm, respectively, and the transverse width 23 mm. Trapezoid lower eyelid clamps were produced in this shape for both right and left. The clamps were used in ten lower eyelid involutional entropion surgeries. We then examined whether the eyelid clamps rotated or not. If there were no rotations, the contour of the new clamps was judged as corresponding to the potential shape of the lower conjunctival fornix. In all cases, the new lower eyelid clamps fitted well, almost without lateral rotation. Therefore, it was demonstrated that the contour of the clamps corresponded to the potential shape of the lower conjunctival fornix.
