Zusammenfassung
Hintergrund
Cocain gehört in Europa zu den am häufigsten konsumierten Stimulanzien. Cocainassoziierte Todesfälle sind weit überwiegend Mischintoxikationen mit Opiatbeteiligung oder im Rahmen von Traumamechanismen unter Einwirkung von Cocain indirekt substanzbezogen. Der toxikologische Stellenwert des Cocains für die Todesursächlichkeit ist deshalb oft schwer zu bestimmen.
Ziel der Arbeit
Es sollte geprüft werden, ob die Cocainkonsumintensität und das Auftreten kardialer pathologischer Veränderungen bezüglich des Todesmechanismus im Zusammenhang stehen.
Material und Methoden
Es wurde die Prävalenz autoptisch/histologisch festgestellter kardialer pathologischer Veränderungen bei Drogentodesfällen mit positivem Cocainnachweis in der Haaranalyse (n = 195, davon n = 43 mit histologisch nachgewiesener kardialer Störung) korreliert mit quantitativen Unterschieden der Cocain‑/Benzoylecgoninkonzentrationen in den Haarproben bzw. gegenüber einer Vergleichsgruppe (n = 151).
Ergebnisse
Bei Verstorbenen zwischen 20 und 40 Jahren zeigten sich zwischen Untersuchungs- und Vergleichskollektiv keine signifikanten Unterschiede der Herzgewichte. Es fanden sich signifikante Differenzen zwischen Herzgewichten, die gemäß dem ermittelten Body-Mass-Index (BMI) erwartet wurden, und den tatsächlich gemessenen. Besonders hohe Cocain‑/Benzoylecgonin-Haar-Konzentrationen (>10 ng/mg) waren jedoch nicht signifikant häufiger mit Epi‑, Peri‑, Myokarditis, Koronarsklerose oder Myokardhypertrophie assoziiert.
Schlussfolgerung
Die relative Bewertung der Kausalität eines Cocainkonsums bei Todesfällen ist nur mithilfe einer interdisziplinären Fallanalyse möglich. Diese hat unter Hinzuziehung aller diagnostischen Optionen zu erfolgen, mit deren Hilfe der Einfluss eines akuten/chronischen Cocainkonsums auf kardiale und zentralnervöse Funktionen eingeschätzt werden kann.
Abstract
Background
Cocaine is one of the most commonly consumed stimulants in Europe. Cocaine-associated deaths are predominantly mixed intoxications with opiate involvement or indirectly related in the context of trauma mechanisms under the influence of cocaine. The toxicological importance of cocaine for the cause of death is therefore often difficult to determine.
Objective
The aim was to investigate whether the intensity of cocaine consumption and the occurrence of pathological cardiac alterations are associated with respect to the mechanism of death.
Material and methods
In the present study the prevalence of pathological cardiac alterations determined at autopsy and histologically in drug-related deaths with positive cocaine detection in hair analysis (n = 195, of which n = 43 with histologically detected cardiac disorders) was correlated with quantitative differences in cocaine and benzoylecgonine concentrations in hair samples and compared to a reference group (n = 151).
Results
Deceased persons between the ages of 20 and 40 years showed no significant differences in heart weights between the study and reference groups. There were significant differences between heart weights which would have been expected according to the body mass index (BMI) and those actually measured; however, particularly high cocaine and benzoylecgonine hair concentrations (>10 ng/mg) were not significantly more frequently associated with epicarditis, pericarditis, myocarditis, coronary sclerosis or myocardial hypertrophy.
Conclusion
The relative assessment of the causal connection of cocaine consumption in drug-related deaths is only feasible by an interdisciplinary case analysis. This includes all diagnostic options to assess the impact of acute or chronic cocaine consumption on cardiac and central nervous system functions.
