Zusammenfassung
Periphere Regionalanästhesien sind für Eingriffe an den Extremitäten allein oder in Kombination mit einer Vollnarkose gut geeignet und werden von Laien als sehr sicher eingeschätzt, obwohl auch hier schwere Zwischenfälle bis hin zum Versterben des Patienten vorkommen können. Die Kasuistik beschreibt den tödlichen Ausgang der Fehllage eines interskalenären Plexuskatheters mit dadurch verursachter i.v.-Injektion des Lokalanästhetikums Ropivacain (Naropin®). Gegen die behandelnden Ärzte wurde ein strafrechtliches Ermittlungsverfahren wegen des Verdachts der fahrlässigen Tötung eingeleitet. Zur Diskussion kommen Komplikationen bei Anlage von interskalenären Plexuskathetern und Intoxikationserscheinungen bei akzidenteller i.v.-Applikation von Ropivacain in Fällen aus der Literatur. Es wird die Möglichkeit von sekundären Katheterfehllagen, auch im Hinblick auf die juristische Würdigung, diskutiert.
Abstract
Regional block anesthesia for surgical interventions on the extremities is commonly used and very well accepted by patients although there is a significant risk of serious incidents including fatalities. This article describes a fatal intravenous administration of ropivacaine via a malpositioned interscalene catheter. A criminal investigation on suspicion of involuntary manslaughter was initiated against the physicians responsible. With reference to the literature on this topic complications with the placement of interscalene brachial plexus blocks and intoxication caused by accidental infusion of ropivacaine are discussed. The possibility of catheter dislocation is discussed also with reference to the legal appraisal.
