Zusammenfassung

Es wird über einen 7 Wochen alten Säugling berichtet, der mit einem Penishämatom und einer Penisschwellung stationär im Krankenhaus aufgenommen wurde. Nach Ausschluss krankhafter Ursachen und fehlender Angabe eines Entstehungsmechanismus durch die Eltern erfolgte 3 Tage später die rechtsmedizinische Vorstellung bei Verdacht auf Kindesmisshandlung. Es wurden ein scharf abgegrenztes Penishämatom und Hämatome in Form von Griffspuren an beiden Unterschenkeln festgestellt. Die folgende Kasuistik soll u. a. die Schwierigkeiten der Differenzierung zwischen einer Kindesmisshandlung und dem Vorliegen eines Haar-Tourniquet-Syndroms aufzeigen.

Abstract

The case of a 7-week-old infant with a hematoma and swelling of the penis is reported. On admission to hospital an infection was excluded and as there no plausible explanation for the injury mechanism was given by the parents a forensic examination was initiated under the suspicion of child abuse. During examination a circumscribed penile hematoma and formed hematomas of both calves were detected. The following case report discusses the difficulties of differentiating between child abuse and penile hair coil tourniquet syndrome.
