Zusammenfassung
Die häufigste Ursache für den plötzlichen Herztod ist die Koronararteriensklerose. Weitere Ursachen schließen zahlreiche kardiale und extrakardiale pathologische Veränderungen ein. Primäre Herztumoren können ebenfalls, in Abhängigkeit von ihrer Lokalisation und Ausprägung, zu Lebzeiten eine Vielfalt an Symptomen bis hin zum Herztod mit sich bringen. Sowohl die klinische als auch die postmortale Diagnose primärer kardialer Tumoren können mitunter schwierig sein und bedürfen zahlreicher, teils spezifischer Untersuchungsmethoden. Die Mehrheit der primären kardialen Neoplasien ist gutartig. Der Anteil der malignen Tumoren wird auf ca. 25–30% geschätzt. Einschließlich der histologischen Subtypen beträgt die Hämangiomrate bei gutartigen primären Herzneoplasien zwischen 0,8 und 5%. Die Autoren stellen ihres Erachtens nach den ersten Fall eines arteriovenösen, intramuskulär gelegenen Angioms vor, bei insgesamt weitgehend unauffälligen makroskopischen Verhältnissen.
Abstract
The most frequent cause of sudden cardiac death is coronary artery disease (CAD). Further causes include various cardiac and extracardiac pathologies. Primary cardiac tumors can induce a variety of symptoms and even lead to cardiac death depending on the location and extent. The clinical and postmortem diagnosis of cardiac tumors can be difficult and requires many, sometimes specific investigation methods. The majority of cardiac tumors are benign with 25–30% being histologically malignant. Only 0.8–5% of all benign primary cardiac tumors are hemangiomas with different histological classification. This article presents a case of an intramuscular arteriovenous hemangioma of the heart without concise macroscopic findings, which is to the best of our knowledge the first case reported in the literature.
