Zusammenfassung

Im Folgenden wird über zwei Fälle großer epiperitonealer Hämatome des M. rectus abdominis nach subkutaner Injektion niedermolekularen Heparins berichtet. In beiden Fällen waren ältere Frauen betroffen, die zur Thromboseprophylaxe für mehrere Tage mit subkutanen Injektionen von Enoxaparin-Natrium (Clexane®) behandelt wurden. Vier bzw. neun Tage nach Krankenhausaufnahme klagten die Patientinnen plötzlich über akute Abdominalschmerzen, für die als Ursache jeweils ein epiperitoneal lokalisiertes Bauchwandhämatom gefunden wurde. Beide Frauen starben an den Blutungskomplikationen. Die Diskussion der medizinrechtlichen Aspekte schließt sich an.

Abstract

A report is given of two cases of large epiperitoneal hematomas of the rectus abdominis muscle following anticoagulant treatment with weight-adapted doses of heparin for subcutaneous injection. Both cases concerned elderly women who had been treated for several days with injections of enoxaparin sodium (Clexane®). The 2 patients suffered from abdominal pain 4 and 9 days, respectively after admission to hospital and a hematoma in the abdominal wall (epiperitoneal) was found. Both women died from intraabdominal bleeding. The medicolegal aspects are discussed.
