Zusammenfassung
Einleitung.
Die Analyse von Panoramaschichtaufnahmen (PSA) der zahnärztlichen Diagnostik für forensische Zwecke ist weit verbreitet. Die Pneumatisation des Jochbogens ("zygomatic air cell defect", ZACD) ist eine anatomische Variante der Hohlraumbildungen des Schläfenbeins, die auf der PSA abgebildet wird. Die Bestimmung von Prävalenz und Charakteristika des ZACD in Kindern und Heranwachsenden wurde bisher nicht beschrieben. Daten zur Prävalenz könnten die gegenwärtige forensisch-odontologische Arbeit in den Anwendungsbereichen "Identifizierung" und "Altersschätzung" unterstützen.
Material und Methoden.
Es wurden die PSA von 1.386 Patienten (Alter: 6–17 Jahren) einer kieferorthopädischen Praxis hinsichtlich des Nachweises eines ZACD untersucht. Die PSA waren zwischen Januar 1998 und Oktober 2001 angefertigt worden. Das durchschnittliche Alter war 11,76 Jahre [Standardabweichung (SD): 2,67 Jahre; Variationsbreite (VB): 6–17 Jahre; weiblich: 760 (54,8%), männlich: 626 (45.2%)].
Ergebnisse.
Ein ZACD wurde auf 31 von 1.298 auswertbaren PSA identifiziert (Prävalenz: 2,38%; weiblich: 15, männlich: 16). Das durchschnittliche Alter der Patienten mit ZACD war 12,56 Jahre (männlich; SD: 2,22 Jahre; VB: 9–15 Jahre) oder 12,33 Jahre (weiblich; SD: 2,38 Jahre; VB: 9–17 Jahre). Der ZACD war überwiegend einseitig ausgebildet (rechts: 13, links: 16, beidseits: 2). Es gab keinen statistisch signifikanten Unterschied der ZACD-Häufigkeit beim Vergleich der Untergruppen der 9- bis 13-Jährigen gegenüber den 14- bis 17-Jährigen; ZACD bei Patienten unter 9 Jahren kamen in dieser Untersuchung nicht vor.
Diskussion.
Diese Untersuchung beweist Pneumatisationen des Jochbogens bei Kindern und Jugendlichen. Die geringe Prävalenz des Merkmals ist in der untersuchten Population ab einem Alter von 9 Jahren nicht verschieden von der Prävalenz bei Erwachsenen. Der Nachweis eines ZACD kann zur Identifizierung von Lebenden oder von Leichnamen herangezogen werden. Außerdem kann er als zusätzliches Merkmal bei Altersschätzungen angewandt werden.
Abstract
Introduction.
The use of panoramic radiographs of the jaws for forensic analyses is well established. The dental findings on the radiographs give valuable information concerning the identity and possible age and can also depict the zygomatic arches. Pneumatized spaces of the zygomatic arch process of the temporal bone are asymptomatic variations known as "zygomatic air cell defects" (ZACD). The analysis of the prevalence and characteristics of ZACDs in children and adolescents has yet not been described and these data might support the current forensic-odontological practice in the fields of identification and age assessment.
Material and methods.
We investigated the panoramic radiographs of 1,386 patients aged 6–17 years. The out-patients were subjected to systematic radiography prior to orthodontic treatment planning between January 1998 and October 2001. The mean age of the 1,386 patients was 11.76 years (SD: 2.67 years, range: 6–17 years) of which 626 were male (45.2%) and 760 were female (54.8%).
Results.
ZACDs were found in 31 out of 1,298 usable radiographs (prevalence: 2.38%; girls: 15, boys: 16). The mean age of patients with ZACD was 12.56 years (boys: SD 2.22 years, range 9–15 years) or 12.33 years (girls: SD 2.38 years, range 9–17 years). Most ZACDs were unilateral (right: 13; left: 16; bilateral: 2) and no ZACDs occurred in children younger than 9 years of age. There was no statistically significant difference concerning ZACDs prevalence in the sub-groups of children aged 9–13 vs 14–17 years.
Conclusions.
This study demonstrates the prevalence of ZACDs in the age group of children and adolescents. The prevalence of this finding in the investigated population did not differ from the prevalences reported for adults, and is low in general. The presence of ZACDs might be valuable for the identification of humans or human remains and for age estimation in addition to other physical and dental findings.
