Abstract
Since Doppler examination where indicated has been introduced into antenatal care in Germany, it is important to evaluate the clinical significance of this diagnostic test in general routine use. For a screening procedure to be effective, there must be a high prevalence of the disease in question and the diagnostic test must be of high quality. A review of the literature shows the efficiency of Doppler ultrasound examination in the detection of pregnancies involving elevated risk of pre-eclampsia and intrauterine growth retardation. With reference to the Cochrane Database, differences in the results of Doppler ultrasound examination of the umbilical artery in non selected pregnant women and in high-risk pregnancies are shown.
Zusammenfassung
Mit der Einführung der Dopplersonographie in die Mutterschaftsvorsorge der BRD bei Vorliegen von Indikationen stellt sich die Frage der klinischen Bedeutung der Untersuchungsmethode bei genereller Anwendung. Vorbedingungen für ein effektives Screening sind hohe Prävalenz der in Frage kommenden Erkrankung und eine hohe Qualität des diagnostischen Tests. Eine Zusammenstellung der Literaturdaten zeigt die Effizienz der dopplersonographischen Untersuchung zur Risikoselektion für die Präeklampsie und die intrauterine Wachstumsretardierung. Die unterschiedliche klinische Bedeutung der Dopplersonographie der A. umbilicalis für das klinische Management wird anhand der Daten der „Cochrane Database” aufgezeigt.
