Abstract
The evaluation of prenatal kidney function by fetal urinalysis for electrolytes, osmolality, proteins, amino acids, urea, creatinine and glucose is reviewed. Interpretation of the data requires knowledge of the normal urine values in healthy fetuses and due consideration for gestational age and maternal and fetal factors. Up to now there is no general consensus in the literature on the normal fetal urine composition. In cases with ultrasonographically detectable oligohydramnios and urinary tract obstruction fetal urinalysis has been proposed as a means of predicting postnatal kidney function. However, the present methods lack sensitivity and specificity, which makes an accurate prognosis in the individual case very difficult.
Zusammenfassung
In der vorliegenden Übersicht wird zur Einschätzung der pränatalen Nierenfunktion mittels fetaler Urinanalytik auf Elektrolyte, Osmolalität, Proteine, Aminosäuren, Harnstoff, Kreatinin und Glukose Stellung genommen. Die Beurteilung der Untersuchungsergebnisse setzt die Kenntnis der Normwerte gesunder Feten in Abhängigkeit von der Schwangerschaftsdauer voraus und muß maternale und fetale Faktoren berücksichtigen. In der Literatur liegen jedoch bisher keine einheitlichen, von der Schwangerschaftsdauer abhängigen Normwerte vor. Bei Feten mit ultrasonographisch festgestellter Oligohydramnie und Harntransportstörung wurde die Diagnostik der Zusammensetzung des fetalen Urins zur Beurteilung der Prognose der postnatalen Nierenfunktion herangezogen. Den bisher verwandten Methoden fehlt jedoch die notwendige Sensitivität und Spezifität, um für den Einzelfall eine gute von einer schlechten Prognose der Nierenleistung mit ausreichender Sicherheit abzugrenzen.
