Hintergrund: Für die Osteochondrosis dissecans gibt es derzeit kein standardisiertes, sondern eine Vielzahl unterschiedlicher Diagnose- und Therapieverfahren. Aus diesem Grund wird in vorliegender Arbeit auf der Basis der aktuellen Literatur und eigenen Erfahrungen eine Momentaufnahme der derzeitigen Therapiemöglichkeiten dargestellt. Therapie: Auf aktuelle Überlegungen zur Ätiologie der Osteochondrosis dissecans tali unter Berücksichtigung neuer biomechanischer Berechnungen folgen die verschiedenen Stadieneinteilungen sowie eine Wertung der bildgebenden Diagnostik. Die Therapiemöglichkeiten beginnen bei der konservativen Therapie, gefolgt von Knorpel schonenden, retrograden Anbohrungen sowie der Stift- oder Schraubenfixation der oft kleinen Dissekate und gehen weiter zu Knorpel ersetzenden Operationstechniken. Bei Knochendefekten oder -nekrosen wird der Einsatz von osteoinduktivem Kollagen vorgestellt. Resümee: All diesen Verfahren ist das Problem der geringen Fallzahlen gemeinsam, sodass es bis heute keine verlässlichen und reproduzierbaren Studien gibt.
Background: In contrast to the well-standardized protocol for therapy of ankle sprain, procedures for the diagnosis and treatment of osteochondritis dissecans (OCD) of the talus vary widely due to its still uncertain etiology. The purpose of this paper is to review the current literature, compare it with our own experience, and include new theories on ankle biomechanics. Therapy: After current reflections on the etiology of OCD taking into consideration new biomechanical calculations, we present different ways of staging OCD and an evaluation of diagnostic means. Treatment begins with conservative therapy, followed by cartilage-saving retrograde drilling and refixation of the often small dissected cartilage with pins or screws, and finally to cartilage-replacing techniques. We introduce the use of osteoinductive collagen for the treatment of bone defects or necrosis. Conclusions: Common to all these procedures, however, is the paucity of cases so that there are no reliable and reproducible studies presently available.
