Biomechanik: Das Sprunggelenk funktioniert nicht einfach durch die bloße Aneinanderreihung anatomischer Strukturen wie Knochen und Bänder, sondern die im Alltag auftretenden Kräfte können nur durch das komplexe Zusammenspiel gelenkführender Strukturen zusammen mit dem neuromuskulären System aufgefangen werden. Ohne dieses neuromuskuläre System, welches nur im lebenden Organismus funktioniert, entstünden Kräfte am OSG, welche weit oberhalb der Belastungsgrenzen von Knorpel und Knochen liegen. Demzufolge können Untersuchungen an Leichenpräparaten nicht unmittelbar auf die Wirklichkeit übertragen werden. Resümee: Die neue Denkweise fordert demnach die Wiederherstellung des gesamten Arthrons inklusive neuromuskulärer Koordination und nicht nur die isolierte Rekonstruktion von Knorpel- oder Bandläsionen.
Biomechanics: The ankle joint does not function by a mere series of anatomical structures such as the bones and ligaments alone. The forces that influence the joint in daily life can only be brought under control by the complex interaction with the neuromuscular system. Disregarding this neuromuscular system which only works in the living organism, forces would appear in the ankle joint that would be far beyond the pressure limit of cartilage and bone. Therefore, results of examinations on cadavers cannot be directly related to reality. Conclusion: The new school of thought demands reconstruction of the whole joint including neuromuscular coordination and not only reconstruction of cartilage and bone lesions.
