Anatomie: Als Verbindung zwischen Fuß und Unterschenkel dient das obere Sprunggelenk der Kraftübertragung auf den Vor- und Rückfuß. Aufgrund der engen topographischen Beziehung der den Fuß versorgenden Leitungsbahnen zum oberem Sprunggelenk besteht bei arthroskopischen Zugängen die Gefahr von Gefäß- und Nervenverletzungen. Im hinteren Bereich ist die Distanz zwischen Hautoberfläche und Gelenkhöhle größer als im vorderen Abschnitt, und die Gefahr iatrogener Schäden ist vergleichsweise höher. Stabilisation und Druckbelastung: Da am Talus keine Sehnen inserieren, besitzt der Bandapparat eine große Bedeutung für die Stabilisation des Gelenks. Außer einer stabilisierenden und führenden Funktion spielen die Sprunggelenkbänder bei der Druckbelastung der seitlichen Gelenkfacetten des oberen Sprunggelenks eine wichtige Rolle. Die das Gelenk bildenden Knochen werden durch die Gelenkresultierende axial auf Druck beansprucht. In den medialen und lateralen Gelenkabschnitten wird der Gelenkdruck vorwiegend durch die Zugverspannung der Bänder erzeugt.
Anatomy: The ankle joint is the connection between the foot and shank which serves to transmit forces to the fore- and hindfoot. All arteries and nerves supplying the foot are in a close topographical relation to the ankle. Stabilization and load transmission: The ankle ligaments play an important role in stabilization of the joint because there are no tendons inserting at the talus. In addition, the ligaments serve an important function for load transmission in the ankle. The articular surfaces of the ankle are subjected to stress along their vertical axis by the resultant force. Bending resulting from tensile forces exerted by the collateral ligaments is superimposed on this compressive stress. The loading from tensile forces is greatest in the medial and lateral areas of the joint where the articular joint stress arises predominantly from the tensile bracing of the collateral ligaments.
