Gelenkinfektionen nach arthroskopischen Eingriffen

Methode: 14 Arbeiten der Jahre von 1982– 2000 mit Beschreibung von 63 Fällen septischer Komplikationen wurden analysiert. Die Latenz vom Eingriff zur Diagnose betrug 3 h bis 4 Monate, meist jedoch 1–2 Wochen. 61 der 63 Infekte manifestierten sich als bakterielle Arthritis, 1 als Gasgangrän und 1 als toxisches Schocksyndrom. Die Diagnose wurde wegen der postoperativen Situation und der häufigen Low-grade-Infektionen oft verspätet gestellt.

Erreger und Prognose: Die Erreger waren in der überwiegenden Mehrzahl Staphylokokken, dabei sehr oft Staphylococcus epidermidis. Clostridien verursachten 2-mal eine septische Arthritis, 1-mal ein Gasphlegmone. Infektquellen waren meist fehlerhaft aufbereitete Instrumente, 1-mal ein übersehener vorbestehender Infekt. 3 Todesfälle durch Staphylokokkensepsis weisen auf die ernste Natur septischer Komplikationen hin.

Method: This paper analyzes 14 papers published from 1982 to 2000 with reports on 63 joint infections following arthroscopic procedures. The time lag between operation and diagnosis varied from 3 h to 4 months. Infections presented as septic arthritis in 61 cases, gas phlegmon in 1 case, and toxic shock syndrome in 1 other case. Diagnosis was often delayed by postoperative symptoms and by the low-grade appearance of the infection.

Causative germs and prognosis: The causative germs were mostly Staphylococci with a high proportion of Staphylococcus epidermidis. Clostridium perfringens caused septic arthritis in two cases and gas gangrene in one case. Instillation of steroids during operation represented the most evident risk factor of infection. Severe sepsis due to Staphylococcus aureus caused three lethal outcomes and demonstrates the serious danger of bacterial arthritis.
