Knorpel-Knochen-Transplantation am oberen Sprunggelenk

Ausgereifter hyaliner Knorpel ist unfähig zur vollständigen Reparation, was die Gelenkchirurgie chondraler Defekte unterschiedlicher Genese bislang sehr erschwert. In den letzten Dekaden wurden verschiedene Operationsmethoden inauguriert, die aber eine sichere und reproduzierbare Heilung hyalinen Gelenkknorpels nicht garantieren können. Mit der Knorpel-Knochen-Transplantation steht erstmals eine Methode zur Verfügung, die zum einen das Potenzial hat, langfristig hyalinen Knorpel im Defektareal zu manifestieren, zum anderen eine Wiederherstellung der Gelenkkongruenz gewährleistet. Im oberen Sprunggelenk bietet sich v. a. die Versorgung von Defekten an, denen eine Osteochondrosis dissecans oder eine traumatische, chondrale Abscherfraktur zugrunde liegt. Aufgrund der guten funktionellen Frühergebnisse werden jedoch auch fokale arthrotische Defekte zunehmend mittels Knorpel-Knochen-Transplantation behandelt. Es werden die operativen Zugangswege, das technische Vorgehen sowie erste Ergebnisse mit dieser neuen Methode am oberen Sprunggelenk beschrieben.

Surgery of various kinds of chondral defects in adults is mostly insufficient due to its inability to regenerate hyaline cartilage. Different surgical methods have been introduced within the last decades, however, no reproducible method is established to allow for complete cartilage healing. Cartilage transplantation based on osteochondral autograft transplantation for the first time has the potential to restore hyaline cartilage within chondral defects for a prolonged period, and enables the surgeon to recreate joint congruency. Osteochondritis dissecans and chondral or osteochondral flake fractures are the preferred indications for osteochondral transplantation. Based on our promising early results, focal arthritic lesions of the ankle joint have also been treated by this method. The operative approaches, technical details, and early results of this new surgical treatment to cure chondral defects within the ankle joint are described.
