Zusammenfassung
Zystische ossäre Läsionen entstehen durch verschiedene Pathomechanismen. Sie bleiben in der Regel klinisch asymptomatisch und können in den meisten Fällen anhand ihrer Lokalisation und typischer radiologischer Merkmale unterschieden werden. Im vorgestellten Fall eines 47-jährigen männlichen Patienten wurde eine subchondral gelegene Zyste der lateralen Femurkondyle, die im postoperativen Verlauf nach durchgeführter VKB-Ersatzplastik auftrat, als intraossäres Ganglion identifiziert. Aufgrund der Größenzunahme der Knochenzyste musste das Einbrechen des darüberliegenden Gelenkknorpels befürchtet werden. Durch die Kombination von autologer Spongiosaplastik mit der Transplantation eines osteochondralen Zylinders konnten eine vollständige knöcherne Auffüllung des intraossären Ganglions und eine niveaugerechte Stabilisierung des Gelenkknorpels erreicht werden.
Abstract
Cystic bone lesions are the result of various pathological mechanisms. They are usually asymptomatic and can be differentiated on the basis of localisation and typical radiological features in most cases. We present the case of a 47-year-old male patient with a subchondral bone cyst of the lateral femoral condyle which was identified as an intraosseous ganglion following ACL reconstruction. Due to the increasing size of the bone cyst, the possibility of fracture to the overlying cartilage could not be excluded. Complete bony healing of the intraosseous ganglion with stabilisation of the surrounding cartilage could be achieved by combined autologous bone grafting and osteochondral autograft transfer system (OATS).
