Zusammenfassung
Die Ruptur des vorderen Kreuzbandes (VKB) ist häufig mit Läsionen der Menisken verbunden. Die Wiederherstellung bzw. der Erhalt der biomechanische Einheit VKB und Menisken zur Rotationsstabilisierung und Knorpelprotektion tritt zunehmend in den Vordergrund. Vor allem bei sehr jungen Patienten mit VKB-Ruptur gilt es die drohende progrediente Arthrose nach Meniskusteilresektion zu verhindern. Dies gilt insbesondere für die nicht seltenen und komplexen Läsionen des Außenmeniskus.
Im Zeitraum September 2003 bis April 2004 wurde bei 74 konsekutiven Patienten im Rahmen einer prospektiv angelegten Studie eine Meniskusrefixation in Kombination mit einer Semi-Grazilis-VKB-Plastik durchgeführt. Nach einem mittleren Nachuntersuchungszeitraum von 53 Monaten konnten 65 der 74 Patienten (88%) klinisch nachuntersucht werden. Bei den 65 Patienten wurden insgesamt 72 Menisken (21 Außenmenisken, 51 Innenmenisken) refixiert.
Bei der klinischen Untersuchung nach über 4 Jahren (51–59 Monaten) zeigten 63 von 72 Menisken (87,5%) keinerlei Meniskussymptomatik und die Patienten waren auch subjektiv beschwerdefrei. Hierbei zeigt v. a. der Außenmeniskus (AM, 95,2%) im Vergleich zum Innenmeniskus (IM, 84,3%) sehr hohe Einheilungsergebnisse.

lnjuries of the anterior cruciate ligament (ACL) are frequently associated with meniscal lesions Reconstruction of the ACL and restoration of rotational stability and repair of meniscal lesions are indicated to ensure long term knee function and avoid subsequent articular cartilage damage. This is particularly important in the young patient with lesions of the lateral meniscus which frequently occur in combination with ACL ruptures.
Between September 2003 and April 2004 in 74 patients meniscal repair was performed in combination with ACL reconstruction using hamstring tendon autograft. After a mean follow-up of 53 months 65 patients (88%) were clinically evaluated. These 65 patients had a total of 72 meniscal repairs (21 lateraI/51 medial).
63 of 72 meniscal repairs (87.5%) had no clinical signs of meniscal instability and had no subjective symptoms more than 4 years postoperatively (51–59 months). Particularly the lateral meniscal repairs showed high healing results with 95.2% compared to sutures of the medial meniscus with 84.3%.
