Zusammenfassung
Die hintere Schulterinstabilität ist im Vergleich zur vorderen nicht nur seltener, sondern auch schwieriger zu diagnostizieren und zu behandeln. Neben den posttraumatischen unidirektionalen hinteren Instabilitäten ohne Hyperlaxitätszeichen finden sich gehäuft atraumatische oder mikrotraumatische unidirektionale Instabilitäten oder multidirektionale Instabilitäten mit posteriorer Hauptkomponente und begleitender Hyperlaxität. Die operative Therapie der Wahl zur Versorgung der hinteren Schulterinstabilität ist bei regelrechten knöchernen Verhältnissen die Kapselshiftoperation mit oder ohne Labrumrepair. Ziel der arthroskopischen Therapie ist es, die minimal-invasiven Vorteile dieses Verfahrens mit den guten und exzellenten Ergebnissen aus den offenen Techniken zu kombinieren. Mit der Einführung neuer arthroskopischer Techniken lässt sich bei genauer Indikationsstellung eine Verbesserung der Resultate der arthroskopischen Techniken verzeichnen. Es scheint, dass sich zum gegenwärtigen Zeitpunkt die Ergebnisse der arthroskopischen Techniken denen der offenen Verfahren annähern.
Abstract
Posterior shoulder instability is less frequent than anterior instability and is also considered more difficult to diagnose and manage. Beside posttraumatic unidirectional cases without signs of hyperlaxity, posterior shoulder instability occurs as either atraumatic or microtraumatic unidirectional instability with hyperlaxity or as the leading component of multidirectional instability with hyperlaxity. The surgical treatment of choice after excluding bony abnormalities includes capsular shift procedures with or without reattachment of the labrum. The goal of arthroscopic treatment is to combine the good results of open techniques with the potential advantages of minimal-invasive surgery. With more modern techniques of suture management and anchor placement, there has been an improvement in the success rate of posterior arthroscopic stabilization. At present, it appears, that the results of arthroscopic treatment of posterior shoulder instability are approaching the results of those achieved through open surgical techniques.
