Zusammenfassung
Ziel der vorliegenden Arbeit war es, die Literaturergebnisse verschiedener Behandlungsstrategien osteochondraler Läsionen am Talus (OLT) miteinander zu vergleichen. Zu diesem Zweck wurden mehrere Datenbanken von 1966 bis Juni 2000 befragt. 39 Studien erfüllten unsere Einschlusskriterien. Randomisierte klinische Studien konnten keine gefunden werden. In den Arbeiten wurden folgende Therapieformen beschrieben: die konservative Behandlung (n=14), das alleinige Débridement (n=4), das Débridement gefolgt von einer Kürettage (n=10), das Débridement, die Kürettage und die Herdanbohrung (n=21), die Spongiosaauffüllung (n=2), die Fragmentrefixation (n=3) und die osteochondrale Transplantation (n=1).
Die konservative Behandlung ergab in 45% der Fälle gute oder sehr gute Ergebnisse. Die besten Resultate der verschiedenen operativen Verfahren wurden nach Débridement, Kürettage und Herdanbohrung erzielt (86% gut und sehr gut), gefolgt vom Débridement und der Kürettage (76%) und dem alleinigen Débridement (38%). Nach diesen Ergebnissen ist eine konservative Behandlung und ein alleiniges Débridement für die Behandlung der OLT nicht mehr zu empfehlen. Die operative Therapie mit Débridement, Kürettage und Herdanbohrung ergab bei der Mehrzahl der behandelten Patienten gute und sehr gute Ergebnisse. Es sind allerdings weitere randomisierte, kontrollierte Studien erforderlich, um die verschiedenen Therapieformen direkt miteinander zu vergleichen.
Abstract
The aim of this study was to compare the results of different treatment strategies for osteochondral defects (OCD) of the talus. Electronic databases from 1966 to June 2000 were systematically screened. Thirty-nine studies fulfilled our inclusion criteria. No randomized clinical trials could be identified.
The results of nonoperative treatment were described in 14 studies, of excision alone in 4, of excision and curettage in 10, of excision, curettage and drilling in 21, of cancellous bone grafting in 2, of fixation in 3, and of osteochondral transplantation in 1.
Good or excellent results were found in 45% of the cases. Comparison of different surgical procedures showed that excision, curettage and drilling resulted in the highest mean success rate (86%), followed by excision and curettage (76%) and excision alone (38%).
From the results of this systematic review we conclude that nonoperative treatment and excision alone are not to be recommended for treatment of talar OCD. Excision, curettage and drilling produced a high percentage of good or excellent results. Further randomized, controlled trials are required to compare the outcome of these two surgical strategies for OCD of the talus.
