Zusammenfassung
Hintergrund
Das Sportklettern und Bouldern hat sich in den letzten 30 Jahren von einer Randsportart zum Breitensport entwickelt, sodass der Sportmediziner zunehmend häufiger mit dem dabei auftretenden Verletzungsmuster konfrontiert wird. Insgesamt ist der Sport relativ sicher, vor allem wenn er in Indooranlagen ausgeführt wird.
Verletzungen
Die Verletzungen treten gehäuft an der oberen Extremität auf, wobei die Finger am meisten betroffen sind. Akut traumatische Läsionen wie Distorsionen, Frakturen oder Bandläsionen sind deutlich seltener als durch repetitive und hochbelastende Kletterbewegungen ausgelöste Überbelastungen. Die häufigste Verletzung ist nicht – wie bei den Erwachsenen – die Ringbandruptur, sondern die Epiphysenfugenstressfraktur der Mittelgliedbasis, welche praktisch nur im Klettersport auftritt. Diese Verletzung kann zwar gut konservativ behandelt werden, beansprucht aber eine lange Heilungszeit von ca. 8 Monaten. Wird die Behandlung allerdings verpasst, können eine Gelenkinkongruenz und Arthrose resultieren.
Abstract
Background
Sport climbing and bouldering has developed from a rarely performed to a very popular sport in the last 30 years. Sport physicians are increasingly faced with its specific and otherwise uncommon injury pattern. Overall, the sport is relatively safe, particularly if performed in indoor facilities.
Injuries
The injuries frequently occur on the upper extremity, with the fingers being the most affected. Acute traumatic lesions such as sprains, fractures or ligament lesions are much rarer than overload and overuse caused by repetitive and highly stressing climbing movements. The most common injury is not the pulley rupture, as in adults, but the epiphyseal stress fracture of the base of the middle phalanx, which occurs practically only in climbing sport. The injury is treated conservatively, has a long recovery time of about 8 months and may lead to joint destruction and arthrosis if missed.
