Zusammenfassung
Hintergrund
Die lumbale Spinalkanalstenose wird durch verschiedene pathologische Veränderungen verursacht. Mit den zur Verfügung stehenden diagnostischen Mitteln soll eine präzise Klassifikation der Erkrankung erfolgen, die eine einheitliche und stadiengerechte Therapie ermöglicht.
Fragestellung
In der vorliegenden Arbeit werden die gängigen Klassifikationen der lumbalen Spinalkanalstenose erläutert und die diagnostischen Möglichkeiten mit Fokus auf die bildgebende Beschreibung von morphologischen Veränderungen dargestellt.
Material und Methoden
Die Arbeit basiert auf einer Literaturrecherche über die letzten 60 Jahre in PubMed sowie eigenen Erfahrungen.
Ergebnisse
Die lumbale Spinalkanalstenose basiert überwiegend auf degenerative Prozesse der Wirbelsäule. Mit MR-tomographischem Bildgebungsverfahren kann eine genaue anatomische Darstellung und damit eine Klassifizierung der Stenose erfolgen.
Schlussfolgerung
Obwohl die modernen Bildgebungsverfahren eine präzise Darstellung der lumbalen Stenosen liefern, trägt die klinischer Symptomatik wesentlich zur therapeutischen Vorgehensweise bei. Mit der anatomischen Klassifizierung kann eine differenzierte operative Dekompression des Spinalkanals geplant werden.
Abstract
Background
Lumbar spinal stenosis is caused by various pathological conditions. With the diagnostic tools available, a precise classification of the condition should be made, which enables a consistent and appropriate therapeutic approach.
Objectives
In the present article, the currently used classifications of lumbar spinal stenosis are discussed and the diagnostic tools are presented, focussing on the imaging descriptions of morphological changes.
Materials and methods
This article is based on a PubMed literature search of the past 60 years and our own experiences.
Results
Lumbar spinal stenosis is caused mainly by degenerative changes to the spine. MR tomographic imaging can result in precise anatomical illustration and classification of the stenosis.
Conclusions
Although modern imaging procedures deliver a very precise illustration of lumbar spinal stenosis, clinical symptoms make a considerable contribution to therapeutic decision-making. With the anatomical classification, differentiated surgical decompression of the spinal canal can be planned.
