Zusammenfassung
Die Kleinzehdeformität tritt häufig in Zusammenhang mit komplexen Vorfußdeformitäten auf. Die Proximale interphalangeale (PIP)-Arthrodese ist ein zuverlässiges Verfahren zur Korrektur einer fixierten Flexionsstellung im PIP-Gelenk. Die mit Abstand am häufigsten verwendete Technik ist die Transfixation mit einem Kirschner(K)-Draht. Allerdings sind K-Draht-Infektionen, Rotationsfehler auf Grund fehlender Rotationssicherung und Irritation des Metatarsophalangeal(MTP)-Gelenks häufige Komplikationen. Die alternative Verwendung einer Drahtcerclage mit einem 0,4-mm-Metalldraht erlaubt die individuelle Einstellung des PIP-Gelenks in einer leichten physiologischen Flexionsstellung. Man kann eine gute Kompression erreichen und Rotationsfehler vermeiden. Zweiunddreißig Patienten bzw. 64 Zehen mit einer fixierten Hammer- bzw. Krallenzehe wurden durch eine PIP-Arthrodese versorgt. Die Nachbeobachtungszeit betrug 2 Jahre. Nach 6 Wochen kam es in allen Fällen zu einer klinischen und radiologischen Durchbauung der Arthrodese. Komplikationen wurden nicht beobachtet. Die guten postoperativen Ergebnisse lassen diese technisch einfache und kostengünstige Methode als eine Alternative zu bekannten Methoden der PIP-Arthrodese erscheinen.
Abstract
Deformities of the small toes often occur with complex forefoot deformities. Proximal interphalangeal joint arthrodesis is a reliable correction procedure for rigid flexed proximal interphalangeal joints. The most often performed technique by far is K-wire fixation; however, pin tract infections, malrotation due to deficient securing of the rotation and irritation of the metatarsophalangeal joints are common complications. By using a 0.4 mm wire cerclage an individual position of the proximal interphalangeal joint in slight flexion is enabled whereby good compression can be achieved and malrotation can be avoided. In 32 patients with 64 fixed claw or hammer toes such a proximal interphalangeal joint arthrodesis was performed. The follow-up time was 2 years. In all cases clinical and radiological osteotomy healing occurred 6 weeks postoperatively and there were no complications. This technically simple and cost-efficient procedure appears to be a good alternative to known proximal interphalangeal joint arthrodesis methods.
