Perioperative management of patients with Parkinson’s disease

Zusammenfassung

Mit zunehmendem Lebensalter steigt die Koinzidenz orthopädischer Erkrankungen mit dem Morbus Parkinson an. Motorische Defizite beim Morbus Parkinson prädisponieren zudem zu orthopädischen Erkrankungen. Operative Eingriffe bei Parkinson-Patienten gehen mit einem höheren Risiko perioperativer Komplikationen einher als bei Patienten ohne Parkinson-Erkrankung. Diese erhöhte Komplikationsrate beruht auf motorischen und autonomen Defiziten des Morbus Parkinson sowie auf Interaktionen zwischen Antiparkinson-Medikation und der anästhesiologischen Medikation. Diese Komplikationsrate kann durch eine entsprechende perioperative Vorgehensweise minimiert werden und stellt heute keine Kontraindikation mehr für operative Eingriffe bei Parkinson-Patienten dar.

Abstract

Ageing patients reveal an increasing coincidence of orthopaedic disorders and Parkinson’s disease (PD). In addition, parkinsonian motor symptoms predispose individuals for orthopaedic diseases. PD patients have a higher risk of perioperative complications than patients without PD; this higher risk is based on motor and autonomous deficits in PD as well as on interactions between antiparkinsonian and anaesthesia medications. The elevated perioperative risk in PD can be reduced by appropriate perioperative management and no longer represents a contraindication to surgery in PD patients.
