Zusammenfassung
Voraussetzung für die normale Entwicklung und Funktion der Hüftgelenke sind physiologische neuromotorische und biomechanische Verhältnisse. Bei neuromuskulären Erkrankungen führen Veränderungen der Muskelaktivität und statische Belastungen zu verschiedenen Krankheitsbildern mit Hüftgelenkinstabilität und Bewegungseinschränkung. Je nach Schwere der Grunderkrankung, Zeitpunkt des Auftretens und Art der Vorbeugung und Behandlung treten in der Folge „Behinderungen“ durch Hüftschmerzen, Subluxationen, Luxationen, Arthrosen, Störungen des Gangablaufs und der Gewichtsübernahme, Schwierigkeiten bei der Sitz- und Lagerungsversorgung oder Probleme bei der Körperpflege auf. Um die Hüftgelenke langfristig schmerzfrei und belastungsstabil erhalten zu können, ist es notwendig, mittels gezielt indizierter bewegungstherapeutischer, medikamentöser oder chirurgischer Maßnahmen eine anhaltende Balancierung der Muskelkräfte im Bereich der Gelenke zu erreichen und eine Dezentrierung mit allmählicher Luxation der Hüftgelenke zu verhindern.
Abstract
Physiologic motor and biomechanical parameters are prerequisites for normal hip development and hip function. Disorders of muscle activity and lack of weight bearing due to neuromuscular diseases may cause clinical symptoms such as an unstable hip or reduced range of motion. Disability and handicap because of pain, hip dislocation, osteoarthritis, gait disorders, or problems in seating and positioning are dependent on the severity of the disease, the time of occurrence, and the means of prevention and treatment. Preservation of pain-free and stable hip joints should be gained by balancing muscular forces and by preventing progressive dislocation. Most important is the exact indication of therapeutic options such as movement and standing therapy as well as drugs and surgery.
